Job overview

We have an exciting opportunity to join the haematology team delivering advanced clinical care to patients with Haematological cancers and none haematological diagnoses. The Advanced Clinical Practitioner post holder will work within the busy and dynamic team covering both the inpatient and outpatient setting based in Macclesfield.

The successful candidate will be highly motivated and driven to improve patient care and experience and will have significant nursing / Allied Health Professional experience within the medical and oncology setting. ÌýThey will have completed an MSc in Advanced Clinical Practice, enabling them to work autonomously in the planning, assessment and delivery of care for this complex group of patients.

The candidate will need to have a high level of experience in hematology and have a sound clinical knowledge, whilst also demonstrating managerial and leadership abilities.

Main duties of the job

  • Provide clinical assessment and support to the care and treatment of a diverse caseload of patients requiring haematology.
  • Demonstrate a highly specialist knowledge and skills in the diagnosis and management of patients with haematological malignancies.
  • Demonstrate a high level of specialist knowledge and skills in the acute care and management of patients receiving SACT.
  • Provide medical support to the day unit as well as the inpatient ward at Macclesfield NHS Trust.
  • Request investigations, procedures and make appropriate referrals maintaining relevant clinical records.
  • Non-Medical prescribing is fundamental in supporting this role
  • Clinical examination skills are essential for this role
  • Provide clinical expertise for developing and enhancing the haematology and transplant service.
  • Work alongside the medical, AHP and nursing teams, maintaining good communication and collaboration between the various multidisciplinary teams.
  • To be a highly visible and approachable role model, ensuring that best practice is disseminated throughout the clinical areas.

Detailed job description and main responsibilities

Advanced Clinical Practice

1.0 Ìý Ìý Ìý Ìý Ìý Plan and manage own workload and the workload of the service, in a flexible manner, ensuring that service provision meets the need of patients.

1.1 Ìý Ìý Ìý Ìý Ìý Recognises and appropriately responds to limitations and gaps in knowledge seeking support where necessary. Demonstrates critical self-reflection, self-awareness, emotional intelligence, resilience, and openness to change.

1.2 Ìý Ìý Ìý Develops and undertakes advanced clinical skills in assessing and managing patients. This includes obtaining informed consent, history taking, screening, systematic clinical examination, requisition and interpretation of appropriate clinical investigations and formulation of clinical management plans.

1.3 Ìý Ìý Ìý Maintain and develop expertise and competence within the field of haematology oncology to enable collaboration with senior colleagues and medical staff, and to facilitate the management of patients.

1.4 Ìý Ìý Ìý ÌýUse evidence-based research to support clinical decisions and management plans.

1.5 Ìý Ìý Ìý Prescribe within the capacity of non-medical prescribing, adhering to Trust policy and existing frameworks including first and subsequent cycles of SACT (as per Medicines Practice Operational Policy July 2010), unlicensed medication and drugs used within clinical trials as qualified Independent Non-Medical Prescriber. ÌýPrescribe appropriate blood products as per Trust policy.

1.9 Ìý Ìý Ìý Liaise with the wider inter-professional teams regarding the management of patients under their care.

1.10 Ìý Ìý Accept referrals from other health care professionals and refer patients directly to other specialties within the appropriate primary, secondary and tertiary care settings.

1.11 Ìý Ìý Communicate highly complex and sensitive information to patients and their significant others, this may include diagnosis, results of scans or other investigations, management options and prognosis.

1.12 Ìý Ìý ÌýWork collaboratively with an appropriate range of multi-agency and multi-professional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings.

1.13 Ìý Ìý Prepare accurate and concise records and reports, both verbal and written. Communicate with the patient’s referring specialist, GP and other community. and hospice-based services at regular intervals as appropriate.Ìý

1.14 Ìý Ìý Contribute to clinical and research governance processes, including adverse incident reporting and the investigation and the management of complaints as appropriate

1.15 Ìý Ìý Involve patients and their carers on issues surrounding health promotion, illness prevention and self-management. Promote and facilitate changes to lifestyle where appropriate.

1.16 Ìý Ìý ÌýContribute to the coordination and effective management of the admission and discharge process as appropriate for disease group with reference to Bed Management Policy. Ìý

1.17 Ìý Ìý Perform clinical procedures safely according to defined protocols. Ìý Ìý ÌýÌý

1.18 Ìý Ìý Develop patient specific information and ensure patients receive appropriate information, advice, and support in relation to their illness and treatment. Provide an environment that encourages patient centered involvement.

1.19 Ìý Ìý Be an active member of relevant Trust committees and groups contributing to developing/implementing local/national strategy.

1.20 Ìý Ìý Collaborate within the Greater Manchester Cancer Alliance and offer clinical support to colleagues within the service, relating to individual patient management, health care delivery and strategic policy initiatives. Assist in the development of integrated patient pathways (e.g., NICE Guidance on Cancer Services, NHS Guidance etc).

1.21 Ìý Ìý Act as an expert resource and provide advice and support to ward/departmental areas and the wider community.

1.22 Ìý Ìý Demonstrate professionalism in terms of attitude, communication, and appearance at all times.

Essential criteria
  • Registered Health Care Professional with clinical responsibilities e.g., Registered Nurse/Pharmacist/Paramedic/Physiotherapist (This list is not exhaustive)
