Job overview
The Integrated Discharge Team is seeking a motivated, friendly and organised individual with experiences working in multi-disciplinary environments to join our team. If you enjoy supporting patients on their journey of hospital discharges, this is the ideal role for you.
This role aims to support the Integrated Discharge Team in facilitating discharges of patients who return home with care needs. You will be responsible for liaising with members from the multi-disciplinary team to support hospital discharges. You will also be supporting administrative duties within the Department.
This is an exciting role as you will have the opportunity to collaborate with a wide range of stakeholders in facilitating hospital discharges of patients. You will be able to develop knowledge and skills in hospital discharges within a complex, fast-paced environment.
You will need to have strong communication, problem-solving, IT and organisational skills, with the ability to meet deadlines as required. This ensures the Department can facilitate hospital discharges efficiently and effectively.
Main duties of the job
- To ensure quality care and support service which enables patients to be supported throughout their care pathway whilst promoting independence and protecting choice, dignity, privacy and safety.
- To work in support of qualified staff and allied health care professionals in the assessment, planning, implementing and evaluation of care without direct supervision of a registered practitioner.
- To work as an associate practitioner in ensuring that holistic care is delivered and standards of care are enhanced
- To always communicate effectively within the team, other health care professionals and with the patient family, carers/ visitors.
- Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.
- To regularly assess evaluate and report on actions and activities to registered practitioner and/or Multidisciplinary Care Team.
- In-reach to local hospitals to support earlier discharges where possible, promoting timely transfers, reducing lengths of stay, supporting wider MDT working.
