Job overview

This is an opportunity to be part of the OAP & discharge team which supports the Capacity & flow for Central Lancashire inpatients Services. Ìý

The post holder will join an established team of practitioners who are part of an exciting service looking to improve discharge arrangements for central patients, particularly those with complex needs requiring funded packages of care and service users in Out of Area Placements (OOA).Ìý This is an opportunity for experienced practitioners to contribute to a developing locality model service.

Main duties of the job

This role will require the post holder to provide clinical management and co-ordination of complex mental health discharge pathways, whilst working collaboratively within a wider multi-organisational team. You will work to developing processes as part of a co-produced service. The post holder will require a working knowledge of commissioning processes.

You will use your experience and knowledge of specialist rehab/urgent care pathways, out of hospital provision as well as needing to influential regarding discharge decisions.

You will promote alternatives to restricted environments whilst working with the patient and the clinical team to increase confidence in the combinatorial impact of multi-agency discharge support, balancing risk and responsibility.

The post holder will also have a role in reviewing the continuing needs of patients at Ormskirk, Skylark (Older Adult), Chorley & Preston Priory (Central Contracted Beds) and any OOA patients from the Central & West locality to ensure that they are progressing through their discharge plan to the least restrictive environment that will meet their needs for safety and service quality.Ìý The post holder will be responsible for making recommendations for patients who are suitable for discharge /stepdown to the community and/or lesser restrictive environment.

Working for our organisation

You will work across Central Locality mental health inpatient units (this includes Ormskirk, Chorley, Skylark & Preston Priory which are part of Central Bed stock) across the Trust footprint and will be required to travel to out of area service providers to undertake review assessments.
