Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which commenced in 2021

An exciting opportunity is available within the Intermediate Care Service (ICS) to work as a Band 6 Discharge Liaison / ICS Nurse.
ICS is a fully integrated service jointly with Walsall Adult Social Care. ICSÌý provides pathways to enable the discharge of patients from hospital and follows them into the community. The team complete comprehensive assessments and care planning. The multi-disciplinary team is made up of Nurses, Social Workers, Social Care Facilitators, Discharge Co-Ordinators, Planners, Therapists, Keyworkers and Administration Staff who work together to facilitate safe and timely discharges from acute hospitals and onto long term care planning.
Main duties of the job
ÌýThe post holder will be expected to work within the multi-disciplinary team to provide senior nurse advice and expertise. They will be responsible for completing comprehensive holistic assessments, care planning and discharge paperwork to facilitate safe and timely discharge. The post holder will also support junior members of the team to promote quality and embed the ethos of the ICS.Ìý Part of the role involves a significant amount of community follow up work, whereby assessments will be undertaken in care homes and patients' own homes post discharge.Ìý
We are looking forÌý a highly motivated registered nurse who is looking for a challenge or to develop their skills in discharge and community pathways. The post holder will beÌý based at Walsall Manor Hospital and a community site and needs to have access to a car.Ìý The successful candidate will be expected to travel around Walsall and the surrounding boroughs.Ìý A good base knowledge of CHC assessment process is desirable but not essential. A knowledge of social services and community pathways would be beneficialÌýto the post
Please note ICS is a seven day service across 365 days a year.

Detailed job description and main responsibilities
To work within the Intermediate Care Services team at a senior nurse level to monitor high standards of care and to facilitate evidence based appropriate/timely discharge underpinned by quality of care.To work within a multi-agency integrated team at a senior nurse level, adopting a ‘no blame’ culture and act as a role model to junior staff.
To assess Patients for Funded Nursing Care (FNC) determinations and
Continuing Health Care (CHC) and oversee / advise on assessments completed by junior nursing staff.
To work closely and build rapport with the Intermediate Care Team and other community based services to support a seamless transition and prevent further admission to acute care.
To support and supervise junior members of the team.
To complete follow up assessments post discharge as part of a multi-agency teamÌý
