Job overview

An exciting opportunity has arisen for aÌý Sister / Charge NurseÌý in Community Nursing, Carmarthenshire.Ìý

The successful candidate will be based within the Intermediate Care Multi- Disciplinary Team (ICMDT) at Eastgate offices in Llanelli and in Priory Day Hospital at Glangwilli Hospital.Ìý

Main duties of the job

The Intermediate Care TeamÌý is responsible for the delivery a high standard of nursing care to patients and clients in their own homes, residential/nursing homes and other care settings in Carmarthenshire.

The successful candidate will be responsible for the shared management of a team of band 2, 3, 4 and 5 nurses, and will be responsible for the supervision, management and teaching of junior staff.

In addition to this they will provide clinical advice to the Team and other Professional agencies, acting as a resource for evidence-based care.

The successful candidate will provide clinical leadership to the Intermediate Care Team in order to meet the needs of the caseload and local population and must be an experienced clinician. You will be responsible for the daily triage of "Patients in a Health Crisis" and be responsible for assessment of accepted patients onto the caseload. As part of this assessment process, you will design a care package and set goals for a team of "Crisis Response, Health Care Support Workers".

The ability to travel between sites in a timely manner is essential for this post.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Qualifications & Knowledge

Essential criteria
  • Registered Nurse (Part 1)
  • Current NMC Registration
  • Bachelor degree (or equivalent experience)
  • Pertinent postgraduate diploma (or equivalent development/experience AND a willingness to undertake appropriate post graduate level study when in post (N.B. Specialist Practice Award in District Nursing required for District Nursing Post))
  • Bronze IQT
Desirable criteria
  • Teaching qualification (or equivalent experience)
  • Recognised management qualification
  • Community Nurse Prescriber

Experience

Essential criteria
  • Appropriate experience within community/specialty and able to demonstrate sound knowledge
  • Able to demonstrate sound knowledge of NMC Code (2015)
  • Experience of working in a multi-professional environment
  • Experience of running a ward/area/department
  • Experience of working with pre-registration nursing/midwifery students
  • Experience of formal/informal teaching and of mentoring pre-registration nursing or midwifery students
  • Understanding of wider nursing and health care related agenda
  • IT skills
  • Supervisory management skills
  • Able to relate theory into practice through reflective skills
