An exciting opportunity has arisen for a Band 5 Community Nurse based in the Narberth & Crymych Integrated Community Team, working from Narberth and/or Crymych Health Centres.Ìý
Ìý
The role will allow you to provide and be accountable for the provision of a high standard of cost effective, safe, individualised nursing care to patients in the community through assessing, planning, implementing and evaluation of patient care.

The post holder will provide care to adults in their own homes, residential or nursing care home, clinic setting or holiday accommodation with any medical/nursing condition.Ìý This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.

Pembrokeshire’s Integrated Community Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.Ìý The post holder will need the ability to travel between sites in a timely manner.

This is a permanent position over a 7 day period to include weekends/bank holidays.

Main duties of the job

The post holder will have delegated responsibility for accepting appropriate referrals onto caseload.Ìý You will carry out an assessment of nursing needs, deciding on the use of appropriate assessment tools to plan appropriate nursing care, deciding on frequency of visits, deciding when care should be reviewed and altering care plan accordingly, in conjunction with the Team Leader.Ìý

You will provide nursing procedures which could include titration of medication in the use of syringe drivers for pain relief in terminal care, use of Doppler ultrasound scanner, performing Venepuncture, disconnection of chemotherapy and care of central and peripheral lines.Ìý

You will be responsible for arranging and giving advice on the use of equipment within the home.ÌýÌý

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications and Knowledge

Essential criteria
  • Level 1 Registered Nurse (Adult)
  • Current NMC Registration
  • Evidence of personal continuing development, statutory and mandatory training
  • Awareness of Health Board policies, procedures and guidelines
  • Ability to relate theory to practice
  • Understanding of primary health care working
Desirable criteria
  • Post registration experience, in acute services
  • Community experience
  • Understanding of clinical governance, health & safety, risk management awareness
  • Teaching and assessing
  • Willing to undertake further training as necessary
  • Evidence of post registration education – Diploma/Degree Level

Experience

Essential criteria
  • Ability to demonstrate sound knowledge of NMC Code (2018)
  • Able to relate theory to practice through reflective skills.
