Our Community First Team has opportunities for Night Community Health & Social Care Assistants to join our team in Derby City.
What does the Community First Team do?
We step in to support people who are discharged from acute hospitals who need an urgent community response.听 You will provide short term/interim support in their homes.听 Our teams are committed to caring and supporting people at home and work over a seven day rota.

What are the hours?
21.45pm - 7.30am听
This includes weekends and bank holidays.
Where would I be working?
Base:听Perth House Assessment Hub and people's own homes.
Area Covered:听Derby City and surrounding area.
Working in this team means you will need be able to meet the travel requirements of going to people鈥檚 homes. This will require you have access to a vehicle. DCHS has a mileage policy and car lease scheme available to support our staff.
Main duties of the job
The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.
To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.
To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the wider multi-disciplinary team.
To carry out individual care without direct supervision in the community and to work in the Perth House Assessment Hub at night to support customers with their rehabilitation goals.听
To provide support to patients to optimise their health and independence.

- GCSE Maths & English (C/4 or above) or equivalent
- Educated to NVQ 3 level (or equivalent) in a relevant subject or equivalent previous proven experience
