Job overview

We have a fantastic opportunity for an Enhanced Clinical Practitioner (ECP) to work in the Swadlincote Acute Home visiting service.

The Acute Home Visiting service works collaboratively with the local GP and Integrated Teams in Swadlincote which provides a varied source of support. Our patient group is patients in their own homes and in the Oakland bedded unit which supports patients discharged from hospital.

Our team is happy, skilled and provide an excellent working environment with opportunities to learn and develop in this role.

You will improve clinical outcomes for patients by enabling them to function independently by increasing their choice to remain in their own home/community and reduce the need for, or prevent inappropriate hospital admission.

Main duties of the job

We aim to recruit clinicians who are passionate in supporting and empowering patients and their family/carers to manage their complex health needs.

We strive to support patients accessing help when needed and optimising health outcomes.

We want to support patients to remain in their own homes as appropriate preventing unnecessary admission to hospital and support patients to return home as needed.

Person specification

Education/Qualifications

Essential criteria
  • RGN or HCPC accredited degree
  • Evidence of post registration clinical professional development
Desirable criteria
  • V300 Independant Non-Medical Prescriber
  • Clinical Decision Making course at Level 7 or equivalent
  • MA level course
  • Teaching qualification

Experience and Knowledge

Essential criteria
  • Knowledge of practical application of clinical governance
  • High level of understanding regarding long term conditions agenda and wider NHS agenda
  • Management of risk and health and safety, plus safeguarding adults and children procedures and ability to implement
Desirable criteria
  • Previous community experience

Skills

Essential criteria
  • High level clinical skills relevant to long term conditions and the management of chronic disease management
  • Excellent communication skills, written and verbal
Desirable criteria
  • Diagnostic and clinical assessment skills
  • Experience of using IT patient records in Primary Care

Other

Essential criteria
  • The post holder will need to have appropriate transport to be able to visit patients in their own home throughout shifts, carrying appropriate equipment.
