Job overview

We are looking for an experienced doctor who is comfortable working within a strong multidisciplinary team to coordinate care for people with complex needs in a community hospital setting.ÌýÌýÌý

University Hospitals Plymouth NHS Trust is a regional trauma centre and one of the largest hospitals located in the beautiful Southwest.

Main duties of the job

  1. Assessment of new admissions to the unit and timely completion of discharge documentationÌýÌýÌý

  1. Review of clinical deterioration in patients at the guidance of nursing staff or medical colleaguesÌý ÌýÌý

  1. Supervise trainees and Trust Doctors in your clinical areasÌý Ìý

  1. Supporting the Senior Doctor team with ward rounds and MDTs – including covering these in event of absence. Ìý Ìý ÌýÌý

  2. The post-holder will work as a member of the multi-disciplinary team, ensuring patient care is based on need with services that are accessible and acceptable to patients and carers. They will be available by appointment to meet with family members and other carers whenÌýappropriate to discuss patients’ management and progressÌý Ìý

  3. Inclusion on theÌýresident and non-resident on call rota based at Mount Gould Hospital. When non-resident on call you are required to be able to attend site within 30 mins of being contacted or complete your on call as resident on call. Ìý

  4. Complete Mortality Screening Tools following in house training Ìý

  5. The post-holder will be expected to uphold the principles of clinical governance and to adhere to the organisation’s clinical governance requirements as outlined in Standards for Better Health. You are expected to be actively engaged in the organisation’s clinical governance programmeÌýand regularly attend governance meetings. Ìý

  6. Your base will be Mount Gould Hospital (MGH)ÌýÌý

Detailed job description and main responsibilities

There are 67 beds across these two community hospital wards for geriatric and post-acute rehabilitation. The case mix is broad and includes those following orthopaedic surgery, trauma and amputation as well as acute physical illness in older people. The majority of patients are admitted from Derriford Hospital following an acute episode of care. There is an experienced multidisciplinary team across these wards including nursing, physiotherapy, occupational therapy, pharmacist, social worker, dietician and psychiatric liaison nurse support.ÌýÌýÌý

Following a successful pilot we have up to 8 End Of Life beds in side rooms across the two wards which are supported by the palliative care team.Ìý

Out of hours cover is provided through the senior nurse on site, supported by off-site hospital wide on-call junior doctor and senior on-call rota (in partnership with Livewell Southwest [LSW]).ÌýÌýÌý

ÌýÌý

There are 6 nursing and medical teams across the two wards. Each patient is seen weekly by a senior doctor, and there is a weekly MDT to discuss all patients. You will be responsible for the medical care for patients across the two wards. You will cross cover absent colleagues too.ÌýÌý

Person specification

Education, qualifications and special training

Essential criteria
  • MB ChB or equivalent
  • Full registration with the GMC
  • MRCP/MRCGP or Diploma of Geriatric Medicine
  • ALS
Desirable criteria
  • Higher degree (MD or PhD)

Knowledge and skills

Essential criteria
  • Extensive experience of Geriatric Medicine Demonstrate evidence of wide based medical knowledge. Demonstrable experience within the NHS Ability to work independently. Ability to work with the wider multi disciplinary team. Ability to undertake Comprehensive Geriatric assessment Ability to manage patients at End of Life and in Last Days of Life Ability to manage patients dementia, delirium and with challenging behaviour Good time management

Experience

Essential criteria
  • Demonstrable experience within the NHS
Desirable criteria
  • Attended appropriate basic specialty courses.

Personal skills

Essential criteria
  • Excellent communication skills with ability to communicate clearly and effectively with patients and staff, both orally and in writing

Practical requirements

  • Ability to work the hours required Ability to cover on call duties. Ability to travel between sites
