Junior Clinical Nurse Specialist Stroke

In conjunction with the Senior Clinical Nurse Stroke team and Stroke Medical Team:

Ìý

·ÌýÌýÌýÌýÌýÌýÌýÌý Respond immediately to acute stroke emergencies within the ED and across all in patient ward areas.

·ÌýÌýÌýÌýÌýÌýÌýÌý Assist in the assessment of stroke emergency cases as to the suitability for thrombolysis/thrombectomy and commence agreed treatment plans.

·ÌýÌýÌýÌýÌýÌýÌýÌý Provide a skilled clinical response involving cannulation, venepuncture, ECGs, observations and medication delivery.

·ÌýÌýÌýÌýÌýÌýÌýÌý Safely navigate and monitor all acute stroke emergencies through the agreed pathway in a timely manner, ensuring all necessary investigations are processed and reviewed, for example transfers to CT/IR/Doppler/MRI.

·ÌýÌýÌýÌýÌýÌýÌýÌý Provide direct specialist nursing care for all stroke patients, including care, information and support for the patients and their next of kin.

·ÌýÌýÌýÌýÌýÌýÌýÌý To work across the pathway by supporting the Acute Stroke Unit as a nurse leader whilst covering the emergency stroke bleep to support the Senior CNS and Medics.

·ÌýÌýÌýÌýÌýÌýÌýÌý Role model competent, skilled stroke care and pathway proven knowledge across UHPT for all patients, their families and our colleagues.

2. MANAGEMENT

·ÌýÌýÌýÌýÌýÌý Uphold and maintain effective working relationships across the MDT.

·ÌýÌýÌýÌýÌýÌý Contribute to the management of the specialist service by contributing to SSNAP data collection and input.

·ÌýÌýÌýÌýÌýÌý Awareness and ability to complete and investigate DATIXs

·ÌýÌýÌýÌýÌýÌý Demonstrable commitment to developing positive working relationships, regarding change management and role development issues.

·ÌýÌýÌýÌýÌýÌý To work with the Senior CNS, ASU Ward Manager and ASU Clinical Educator to provide continued staff education and support/appraisals.

·ÌýÌýÌýÌýÌýÌý Be a point of advice and support for the development of junior staff.

·ÌýÌýÌýÌýÌýÌý Always demonstrate compliance with policies and procedures, working to both local and national guidelines.
