Are you an Occupational Therapist with a passion for fast-paced environments looking for a unique development opportunity in West Essex?

This role is open to band 5's looking to develop and may not feel ready for a six but this post will enable you to join as a 5 to then progress to a band 6 with a clear support and development plan.听

If so, this is a fantastic development opportunity for you. In partnership with ECC, EPUT, and PAHT, we developed an innovative integrated rotational programme that offers work experience in Adult听Social Care (ECC), community services (EPUT), and Acute (PAHT).

The rotational programme is designed for ambitious professionals interested in widening their career development in various aspects of OT and will last just over two years. In this role, you will play a vital role in assessing and treating patients with a wide range of conditions. As part of the integrated partnership, you will work closely with other healthcare professionals and be integral in providing comprehensive care and supporting patients.

The programme is separated into four 9-month consecutive placements (Urgent Care Response, stroke/early supported discharge, Adult Social Care and Acute Services), finishing with a permanent position as a member of the AHP EPUT team.

Main duties of the job

飩 Conduct comprehensive occupational therapy assessments to identify patients' functional needs and goals.

飩 Develop and implement individualised treatment plans that focus on improving patients' ability to perform activities of daily living (ADLs), such as dressing, bathing, and eating and support them to remain safe and independent in their own homes and support recovery.

飩 Provide education and training to patients and their families on how to manage their condition and prevent future complications.

飩 Collaborate with other healthcare professionals to ensure that patients receive coordinated and seamless care.

飩 Contribute to the service as a whole with MDTs, service development, supporting the trust and attending service meetings.

飩 Work autonomously during the shift, feeding back to Clinical Leads within UCRT.

飩 Document all assessments and interventions in a timely and accurate manner.

The post holders will need to have a high degree of professional autonomy and be able to make critical/clinical judgements using expert knowledge, skills and experience in order to provide a gold standard of holistic care, including complex management to patients with both physical and mental health conditions.

Detailed job description and main responsibilities

The post holder will work autonomously and in collaboration with other healthcare professionals and statutory and voluntary agencies to deliver excellent standards of patient care for patients requiring support in planned care.

Clinical responsibilities
飩 Undertake comprehensive, holistic assessments of individuals, including those with a complex presentation, using investigative, analytical & clinical reasoning skills, and formulate a treatment plan.
飩 Undertake functional assessments for patients by using investigative and analytical skills for those with acute/complex/rehabilitation or palliative care needs
飩 Liaise with GPs, social services, inpatient teams, other allied health care professionals, and the voluntary sector to ensure identified needs are met and care is coordinated appropriately.
飩 Communicate effectively with individuals and their carers to maximise potential and to aid motivation and understanding throughout the treatment process.
飩 Empower/enable patients and carers to maximise the potential for self-care.
飩 Required to use tact and persuasive skills in order to gain the individual鈥檚 cooperation in their treatment plan.
飩 Be responsible for ensuring workload for self and others is planned and prioritised according to service targets and needs.
飩 Ensure patient-held records are completed for each visit and that SystmOne inputting is completed on a daily basis.
飩 Where safe care is not able to be sustained to liaise with the broader community services and care coordination centre to plan and implement appropriate actions

Qualifications

Essential criteria
  • Recognised Physiotherapy or Occupational therapy degree/qualification
  • Health Care Profession Council Registration as a Physiotherapist or Occupational therapist

Skills and Experience

Essential criteria
  • Broad range of clinical experience including community, rehabilitation and admission avoidance.
  • Experience of working autonomously and managing a varied caseload
  • evidence of supporting peers or junior staff
