Across Gloucestershire we have had major investment in our Home First and Reablement Team and as a result of this we have an opportunity for a Band 6 Physiotherapist to join our Team and be a key part of developing our therapy led service in the South Cotswolds. Our Team is based in Cirencester, and we work closely with the GP practices in the Primary Care Network, alongside our Adult Social Care colleagues and voluntary organisations in the locality.

Main duties of the job

In line with the Government hospital discharge and community support policy, and the discharge to assess model, our Home First service is a time-limited assessment service for people in their own homes to facilitate rapid discharge from Acute and Community hospitals.

We also provide Reablement to patients in the community, to prevent hospital admission and to facilitate maximum independence, working on jointly agreed goals, promoting personalised care.

Detailed job description and main responsibilities

As a Physiotherapist you would be a key part of our team, providing assessment, goal planning and reviews, working alongside Occupational Therapists, Nurses, Reablement workers and rehabilitation assistants to enable our patients to gain their maximum level of independence in their own home. The caseload will be varied, ranging from rapid assessment to prevent hospital admission, Home First assessments to establish pathways of care following hospital admission, through to setting goals as part of a reablement pathway.

QUALIFICATIONS

Essential criteria
  • Diploma / Degree in Physiotherapy
  • HCPC registration
  • Driving licence and use of vehicle
Desirable criteria
  • Membership to a relevant professional body e.g. CSP

LENGTH AND / OR NATURE OF EXPERIENCE

Essential criteria
  • Broad range of clinical experience across a variety of clinical areas
  • Solid experience as a physiotherapist, or the ability to demonstrate relevant experience in the field of rehabilitation
  • Experience of managing junior members of staff and rehabilitation assistants
  • Evidence of working unsupervised
  • Evidence of working across multi-disciplinary and multi-agency boundaries
Desirable criteria
  • Experience of performance management

PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE

Essential criteria
  • Able to deliver community rehabilitation and treatment of patients with complex clinical presentations
  • Ability to deliver in-service training programmes
  • Ability to clinically practice the core areas of exercise therapy, management of pain relief, mobility training and therapeutic handling
  • Able to demonstrate specialist knowledge in rehabilitation of the patient with complex needs / multi-pathologies
