Main duties of the job

Coordination of team activities to ensure that the NHS All Age Continuing Care processes run efficiently and within mandated timescales

Act as the first point of contact for internal and external stakeholders, ensuring queries are directed appropriately.

Monitor the commissioning team inbox, ensuring prompt responses to enquiries and efficient allocation and escalation.

Supporting the team with the management of projects, gathering information and undertaking enquires as and when is necessary

Assist in collating and preparing data for provider performance reviews and audits.

Schedule, organise, and minute meetings, ensuring follow-up actions are assigned and tracked.

Support the team with documentation for contract setup, renewals, and compliance monitoring.

To communicate on a daily basis, both verbally and in writing with team colleagues (clinical and non-clinical), care providers and other health and social care agencies, providing and receiving sensitive information.

Develop good working relationships with care providers, Local Authority Colleagues and colleagues from other NHS bodies.

Detailed job description and main responsibilities

All Age Continuing Care covers, Continuing Healthcare and NHS-funded Nursing Care as aÌý
statutory duty of the Integrated Care Board for individuals with a ‘primary health need’,Ìý
including Mental Health, Learning Disabilities, Physical Disability, and Fast Track.Ìý
All Age Continuing Care also has Integrated Care Board duties to children and young peopleÌý
(Children’s Continuing Care), joint packages of health and social care (where an individual isÌý
not eligible for Continuing Healthcare but specific needs have been identified through theÌý
Decision Support Tool that are beyond the powers of the local authority to meet on its own),Ìý
personal health budgets, stand-alone health needs, s117 of the Mental Health Act andÌý
Specialist Rehabilitation (Acquired Brain Injury) .
All Age Continuing Care delivers the patient specific Learning Disabilities and AutismÌý
Programme Integrated Care Board responsibilities.

Person specification

Knowledge, Training and Experience

Essential criteria
  • Educated to GCSE/O level grade C in English or equivalent
  • Educated to NVQ 3 level in Business Administration/Management or relevant subject or equivalent level of qualification or significant equivalent previous proven experience.
  • Significant administrative experience including maintaining office systems.
  • Demonstrable experience in dealing with the public and dealing with sensitive and confidential information.
  • Intermediate knowledge of IT systems and software programs such as Outlook, Word, Excel, PowerPoint and Access.
  • Experience of minute taking.
  • Experience of the application of the Confidentiality and Data Protection Act.
  • Demonstrable supervisory experience of a small team.

Communication and Relationship Skills

Essential criteria
  • Clear communicator with excellent telephone, writing, data entry and presentation skills: capable of constructing and delivering clear information/ instructions to staff and service users.
  • Ability to work under pressure in a busy working environment and able to multi-task.
  • Excellent interpersonal and negotiation with the ability to deal sensitively with patients and their families/representatives.
  • Effective team working
  • Ability to demonstrate the ability to deal with difficult/challenging situations.
