Clinical Nurse Specialist for Older Persons in Care Homes

Job overview

We are looking for enthusiastic, creative, progressive and adaptable individuals to be part of our future developments in supporting the care of older people across our urgent care pathways from acute hospitals to care homes.

As a Clinical Nurse Specialist for Older People you will need to demonstrate a good understanding of how to manage the complex care needs of older people and be willing to develop the service in line with the Enhancing Care in Care Homes framework, and work collaboratively to support the PCN s to enable the care homes to feel part of the community health family.
Having excellent assessment and communication skills and being able to articulate key information to patients and families is a key requisite for the role, as the CNS role supports the Pathway 3 assessment process from the Hospital Discharge and Community Support; policy and operating model. Main duties of the job

As a Band 7 Clinical Nurse Specialist you will use your clinical experience, expertise and advanced skills to provide leadership and effective caseload management, working closely with users of our services and other stakeholders. Band 7 Clinical Nurse Specialists are expected to be kind and responsive but professional and informative and contribute to the quality of the services provided across the health economy by:
• Keeping the people who use our services as safe as possible through the use of sound clinical skills and effective risk assessments
• Ensuring the best possible clinical outcomes by using up-to-date skills and using evidence-based policies and procedures
• Ensuring the people using our services have a good experience by respecting, empowering and working in partnership with everyone involved in care planning.

Person specification

QUALIFICATIONS

Essential criteria
  • 1st Level RGN
  • Mentoring qualification
  • Evidence of continued professional development
  • Evidence of Adult Protection training at least at level two
  • Minimum five years working at a senior level in a variety of community settings
  • Willingness to study at Masters level
  • Driving licence and access to car
Desirable criteria
  • Masters level qualification in a relevant area of practice to the post.
  • E.N.B. 941 or evidence of formal study within the Care of the Older Person/ Older Person Mental Health
  • Evidence of formal courses in End of Life Care, Quality Assurance, Performance monitoring and standards or Root Cause Analysis

EXPERIENCE

Essential criteria
  • Experience of change management or workforce development
  • Experience of clinical leadership in a multi- agency arena
  • Teaching and training experience, including presentation & facilitation skills
  • Experience with Quality Frameworks, Clinical Benchmarking and their use in Assurance.
  • Experience within Clinical Audit, Research and Development
Desirable criteria
  • Independent sector experience, especially within CH(N)
  • High level of clinical expertise evidenced within clinical practice
  • Experience of educational needs analysis, formulation, delivery and evaluation of training and development programmes.
  • Experience in the use of Essence of Care and Self Assessment Quality Frameworks

KNOWLEDGE

Essential criteria
  • Highly developed specialist professional knowledge on a wide range of nursing issues impacting on the care of the older people
  • Good working knowledge of computers and office based software.
  • Current knowledge of a wide range of clinical procedures to support, patient outcomes and the training and educational element of the post.
  • A full understanding of NHS Continuing Care and Funded Nursing Care.
  • working knowledge and understanding of the Safeguarding Vulnerable Adults Agenda and its related areas of mental capacity and deprivation of liberty.
  • robust understanding of Nursing, Governance, Quality Assurance and Performance Management.
  • Knowledge of National and local policies/frameworks impacting on this area of care.
Desirable criteria
  • In depth knowledge of the impact of the Care Quality Commission in the regulation, safety, quality and standards for the Care Home sector.
  • Knowledge of local commissioning strategies to promote a healthy, responsive, quality and safe market.
