Job overview

Our intermediate role bridges between the community diabetes services and hospital diabetes services of West Kent, with a focus on education, diabetes technologies, supporting discharge and reducing avoidable admissions.聽

The ideal candidate would be a experienced diabetes specialist nurse, or diabetes pratitioner currently working in primary / community care or a complex care nurse with diabetes responsibilities.

The role requires the ability to work independently to a high standard, be highly self motivated, organised and enthusiatic about the delivery and organisation of diabetes care between primary and secondary care.聽

This role supports smooth transition of patients to there own homes and nursing / residential homes that might need access to diabetes technology and education to support this transition.聽 This role also provides priority reviews for type 2 patients that have had delayed community review.

We are looking for an innovative, ambitious and forward thinking diabetes nurse to help us drive improvements within service delivery.聽聽

A driving licence is essential for this role.

We are particularly interested to hear from Diabetes Specialist Nurses or Practice nurses with community and/or education experience.聽聽

Main duties of the job

We are a friendly team and a team who would support anyone new to our service.

You will act in this role as a specialist resource and provide specialist advice in relation to care of people with diabetes; providing education and training in a variety of settings.聽

Detailed job description and main responsibilities

Once your pre-employment checks have been completed you will be contacted by Recruitment to arrange your start date which will be in line with one of our Corporate Inductions.聽

Following your Corporate Induction you will be expected to attend the Trust鈥檚 2-week Induction Programme for RNs, NAs, RMs, ODPs and Paramedics. 聽The programme is full-time (please email [email protected] if this is problematic) and will involve travel to both Trust sites. The programme includes supernumerary shifts in the clinical area, classroom-based sessions and e-learning.

Please note that appointment to this post will be subject to a satisfactory Disclosure & Barring Service Check at Enhanced Level. (Enhanced level now includes regulated checks against vulnerable adults and children)

Qualifications

Essential criteria
  • 鈥 Level 1 NMC Adult Registered Nurse
  • 鈥 Relevant post registration qualification
  • 鈥 Diabetes Diploma/Certificate
  • 鈥 Teaching qualification or equivalent expert experience
  • 鈥 Non-Medical Independent Prescriber / or willingness to undertake within 12 months.
Desirable criteria
  • 鈥 Mentorship qualification or willingness to undertake
  • 鈥 Counselling qualification or willingness to undertake
  • 鈥 Non-Medical Independent Prescriber/ Willingness to undertake.
  • 鈥 DAFNE qualification
  • 鈥 Experience of managing diabetes in specialist clinics (renal, pregnancy / gestational/pump, YAC

Experience/Knowledge

Essential criteria
  • 鈥 Minimum 3 years post registration experience (full time hours)
  • 鈥 Extensive knowledge of diabetes having previous experience in a DSN role for minimum of 12 months.
  • 鈥 Critical application of detailed knowledge in the field of diabetes.
  • 鈥 Diabetes ward / practice experience and a willingness to develop using the Trend Career competency document.
  • 鈥 Minimum 3 years post registration experience.
  • 鈥 Extensive knowledge of diabetes having previous experience in a DSN role for minimum of 12 months.
  • 鈥 Critical application of detailed knowledge in the field of diabetes.
  • 鈥 Diabetes ward / practice experience and a willingness to develop using the Trend Career competency document.
  • 鈥 Ability to work independently and as part of a team
  • 鈥 A working knowledge of the management of Type 1 & Type 2 Diabetes
