Are you a qualified and registered Nurse, Social Worker or Occupational Therapist seeking a new and exciting challenge?

We are recruiting 2 band 6 practitioner to help support the ongoing development of our Integrated Discharge Team. The post holder will be expected to meet service delivery working Monday - Friday, 0900-1700.

These posts will be based at The Harbour, Blackpool.

This is an interesting development opportunity for experienced practitioners to join a team refining the discharge pathway for patients, particularly those with complex needs requiring funded packages of care.

You will provide clinical management and co-ordination of complex mental health discharge pathways working collaboratively within a wider multi-organisational team as part of co-produced service developments. The post holder will require a working knowledge of commissioning processes.

Main duties of the job

The post holder will use their experience and knowledge of specialist discharge pathways and out of hospital provision to influence discharge decisions; promoting the alternatives to restricted environments and working with the patient and the clinical team to increase confidence in the combined impact of multi-agency discharge support, balancing risk and responsibility. The post is largely ward based supporting the ward Multi-Disciplinary Teams around effective discharge planning. Service need is across a number of our LCFT wards and the post holder may be required to travel across LCFT hospital sites.

You will have a role in contributing to and reviewing the discharge plans of patients to ensure that the patient is progressing (including rehabilitation) to the least restrictive environment that will meet their needs for safety and service quality. You will be responsible for making recommendations for patients who are suitable for transfer to a less restrictive environment and will support the Care Coordinator with this process.

Education

Essential criteria
  • Relevant professional registration for example; RMN, Social Worker or OT.
  • Educated to Degree Level in appropriate subject or equivalent.
  • Several years post qualifying experience

Knowledge

Essential criteria
  • In depth knowledge of mental health clinical practice and procedures.
  • Management skills
  • Expert skills in risk management, patient safety, prioritisation

Experience

Essential criteria
  • Experience of operational management at Team Leader/ Deputy Team Leader/or clinical lead
  • Demonstrable experience of working in front line clinical service and understanding of pressures and ways of working.
  • Community Experience also essential
