This innovative post will provide the Postholder with a unique opportunity to work within a multidisciplinary team supporting people to age well.
Working as part of a multi-disciplinary team, the Postholder will work flexibility across a wide range of services within the community, community and intermediate care beds in addition to supporting safe discharge form hospital.
The Post holder will support where appropriate a rapid response function to all setting within the community, enabling continued management of patient within a community setting, preventing avoidable admission/attendance in secondary care.
This role will provide medical advice and guidance to community care colleagues.
The Post holder will work with clinical managers to ensure that services are delivered in accordance to national quality standards.
An Enhanced DBS with Children鈥檚 and Adults' Barred List checks will be obtained during pre-employment checks.
This post involves contact with patients and families face to face within a community setting. Therefore it cannot be ruled out that the postholder may come in to contact with people who have COVID-19, although the Trust provides PPE and takes all reasonable steps to minimise risk.

Main duties of the job
To work flexibly across a wide range of service timetables within a community care setting, providing face to face consultation sessions, clinical sessions, telephone triage, home visits, case management and clinical administration.
To assess, investigate and (where appropriate) undertake clinical reviews, for all referrals to CRT/Intermediate Care Community Services and community beds including frailty assessment.
To review and action diagnostic test promptly.To provide medical support/advice to other primary and community care colleagues. Utilising and asset based approach and promotion of self management to support ongoing care.
To promote and implement 鈥渆nd of life鈥 strategies (Gold Standard Framework etc.), delivering high quality palliative care. To facilitate and support advance care planning including ceilings to care plans
To provide medical input and cover to virtual wards, community beds/residential nursing, EMI care homes, IMC beds, Daily ward round for community beds and home visits.
To work closely with other primary and community care colleagues in locality hubs/teams to deliver high quality patient centred clinical care. This will include where appropriate attendance/participation in multi-disciplinary team meetings (MDTs).
The service is听 operational seven days a week听 and own transport is essential for the ability to travel across the borough

Detailed job description and main responsibilities
Key results from the job holder
- To work flexibly across a wide range of service timetables within a community care setting, providing face to face consultation sessions, clinical sessions, telephone triage, home visits, case management and clinical administration.
- To work flexibly providing cover for leave arrangements.
- To prescribe in accordance to the national formulary for all patients and where it is deemed necessary to prescribe outside the national formulary, to ensure clinical rationale is fully recorded in patient notes.
- To assess, investigate and (where appropriate) undertake clinical reviews, for all referrals to CRT/Intermediate Care Community Services and community beds including frailty assessment.
- To assess/treat the patient in accordance to agreed clinical pathways
- To review and action diagnostic test promptly.
- To support the establishment of links and referral processes/pathways within secondary or specialist care, in line with local polices and protocols.
- To facilitate safe & effective handover practice in accordance to service operational procedures.
- To provide appropriate self-management and preventative health care advice to all ages and level of health needs.
- To provide medical support/advice to other primary and community care colleagues.
- Utilising and asset based approach and promotion of self management to support ongoing care.
- To promote and implement 鈥渆nd of life鈥 strategies (Gold Standard Framework etc.), delivering high quality palliative care. To facilitate and support advance care planning including ceilings to care plans.
- To interpret urgent diagnostic investigation such as ECG, X-Ray and Blood Tests.
- To provide medical input and cover to virtual wards, community beds/residential nursing, EMI care homes, IMC beds, Daily ward round for community beds and home visits.
- To work closely with other primary and community care colleagues in locality hubs/teams to deliver high quality patient centred clinical care. This will include where appropriate attendance/participation in multi-disciplinary team meetings (MDTs).
- To participated in long term condition management with appropriate investigation, recall and referral to specialist clinics.
- To prioritise work and deliver appropriate case management care.
- To work as part of a team to ensure operational targets are met.

Responsibility for Health & Safety

Qualifications
Essential criteria
- Full Registration with the GMC as General Practitioner and hold valid licence to practice
- MB, ChB , MRCGP or equivalent certificate of prescribed/equivalent training in general medicine
- Member of the Royal College of General Practitioners
- Joint Certificate of Vocational Training or equivalent
Desirable criteria
- ILS or ALS certificate
Experience
Essential criteria
- Post registration Primary Care experience
- Evidence of annual GP Appraisal Revalidation
Desirable criteria
- Demonstrate experience of working within an OOH or within a GP rota covering service
- Experience of pallative care
- Experience of paediatrics care
- Experience of A+E Avoidance
- Teaching and training of other Health professionals
Skills
Essential criteria
- Computer literate and familiar with how patient management systems work
- Demonstrate skills in written and spoken English adequate to enable effective communication about medical topics with patients, relatives and staff.
Knowledge
Essential criteria
- Understanding of the principles of evidence based medicine rationale prescribing
Desirable criteria
- Knowledge of the national quality requirements in the delivery of out of hours service
- Knowledge of clinical coding
- Knowledge of SystmOne electronic patient management system
