The Hillingdon Podiatry team has a large High-risk caseload across the borough and is the main provider for the Multidisciplinary Foot team based at Hillingdon Hospital. This is a Band 6 Clinical Podiatrist role and we will be willing to consider a newly qualified Podiatrist for this role.

Extensive Community experience is not essential but you will need to demonstrate that you have an understanding and awareness of autonomous working in the community setting.

Main duties of the job
To be part of a highly motivated team. Working independently in a variety of community, domiciliary settings, providing a high standard of care.
To assess, diagnose, develop and implement individualised care plans for patients with a wide variety of clinical needs, including acute, chronic neuropathies, chronic diseases, vascular problems and wound management.
The post holder will be expected to plan and manage their own cases, which will include providing specialist care for patients with Diabetes, Rheumatology, Peripheral Vascular Disease and Biomechanical problems, following clinical pathways and sign posting to appropriate onward referrals.

Detailed job description and main responsibilities

1.听听听 To be responsible/accountable for the assessment diagnosis, planning, implementation and evaluation of Podiatric care to a variety of patients with a wide range of clinical needs:
听
2.听听听 This includes:听 patients with medical condition that affect the feet such as patients with Diabetes at different risk categories, Chronic Neuropathy; Chronic diseases including; e.g. Rheumatoid听听 Arthritis; Vascular problems, Terminally ill patients and patients with Physically/Mental Health/Learning Difficulties.
听
3.听听听 To be responsible for the assessment of new patients with subsequent development of individual care packages and provision of the appropriate footcare education advice to patients
听
4.听听听 To be responsible for the assessment, diagnosis, planning, implementation and evaluation of Podiatric Care to a variety of patients with a wide and high range of clinical needs including: Biomechanical conditions听

5.听听听 Able to carry out a foot ulcer risk assessment and ulcer management, including the identification of vascular insufficiency, neurological deficit, significant foot deformity, trauma or increased foot pressures.
听
6.听听听 Understanding of the wound healing process and its potential complications and carryout sharp debridement wounds, wound bed management and use of appropriate dressing.听 Recognise sign and symptoms of wound infection and manage appropriately. To recognises the need, and refers the patient, for advanced management.听
听
7.听听听 To preform basic biomechanical examination, produce temporary insoles and refer appropriately.听
听
8.听听听 To perform comprehensive assessment of the diabetic/high risk foot including: providing a听
full annual review assessment to including sensory, vascular analysis (ABPI鈥檚 as required).听听
听
9.听听听 To Understand an refer appropriately for investigations as required e.g. microbiology, radiology
听
10.听 To use the decision-making process to identify when patients require the intervention of other professional and/or departmental specialist areas by following agreed departmental referral pathways e.g. acute wound care/biomechanics.
听
11.听 To participate/carry out nail surgery.
听
12.听 To be actively involved in the provision of Foot Health Education advice to patients, relatives and carers
听
13.听 Care will be provided mainly in a range of primary care settings, involving working on your own in community clinics, patients鈥 homes, ward rounds, hospital setting and residential homes for the elderly.听听听
听
14.听 To have the capability to move and handle patients who may be immobile, wheelchair or bed-bound often in confined spaces.
听
15.听 To have highly developed physical skills, precision hand/eye co-ordination which involves highly skilled debridement techniques of acute/chronic, neuropathic and ischaemic wounds, often for long periods.
听
16.听 To follow departmental Policies Procedures and Guidelines but have the discretion to make own clinical diagnosis and decisions within professional boundaries.
听
17.听 To undertake other work or duties across the whole service, as directed by the Team Leaders.
听
18.听 To actively be involved in support student/ apprentice training and staff from other disciplines.
听
19.听 To support Footcare Assistants and band 5 Staff.
20.听 To be flexible and provide cover for other Podiatrists at short notice or during periods of absence.
听
21.听 To implement in day to day work Trust values in delivery care to the Trust diverse population.

Qualifications
Essential criteria
- HCPC Podiatrist Registration
- Degree or Diploma in Podiatric Medicine
- Relevant experience
- Evidence of CPD Portfolio
- Hold a Certificate in local anaesthesia.
Experience
Essential criteria
- Experience in the NHS working
- Experience of treating a varied caseload of podiatry patients, including those with high-risk conditions
- Experience of working in both a team and on your own managing a caseload with the ability to 鈥榬isk categorised patients
- Experience of multi-disciplinary working, care in the community and domiciliary settings
- Clinical skills including, vascular and neurological assessment.
- Demonstrate clinical reasoning and decision-making skills in the management of podiatric conditions including care planning and evaluation.
- Ability to undertake nails surgery assessments, procedure and postoperative care.
- Knowledge of health and Safety at work
Skills and Knowledge
Essential criteria
- Knowledge of the effect of diabetes in podiatric care
- Sound knowledge and experience in wound care including treatment modalities and Biomechanics
- Demonstrates current clinical knowledge regarding evidence base and best practices
- Ability and knowledge of appropriate investigations required In the treatment of the high -risk foot
- Knowledge of national policy relating to the diabetic foot
