Job overview

Part of the Devon Cluster, HMP Exeter is a category B men’s reception prison with a small resettlement function, holdingÌýapproximately 300 prisoners. The post includes an interesting and varied range of clinical responsibilities for prisoners including severe and enduring mental illness, neurodiversity, personality disorders, learning disability and head injuries. The work entails close multidisciplinary working within the mental health team and alongside the substance misuse and primary care teams in the prison. You will liaise with prison staff and probation and provide input into MAPPA meetings and pre-release meetings with community and in patient teams and probation.

The post holder will be Section 12 registered and appointed to a part-time, 8 PA role, providing cover for HMP Exeter Ìýmental health services and supporting the full Oxleas service through Senior leadership on site. Following appointment there will be a meeting at no later than three months with the Clinical Director and Service Manager to review and revise the job plan and objectives of the post holder and agree the formal job plan. The job plan will be reviewed at least annually thereafter.

The overall split of the programmed activities is 6 to be devoted to direct clinical care and 2 to Supporting Professional Activities.

There are no out of hours on-call associated with this post.

Main duties of the job

Clinical work - HMP Exeter

·ÌýÌýÌýÌýÌýÌý Assessing and reviewing prisoners; and working in collaboration with MDT and prisoners to formulate management plans that is then incorporated into individualised care plans.ÌýÌýÌý

·ÌýÌýÌýÌýÌýÌý Liaising with services outside the prison to arrange CPAs, aftercare plans or transfers to hospital.Ìý

·ÌýÌýÌýÌýÌýÌý Providing expert advice and guidance for individuals with complex mental state and risk presentation.Ìý

Ìý·ÌýÌýÌýÌýÌýÌý Involved in quality improvement initiatives, to be agreed with multidisciplinary colleagues and the supervising ConsultantÌý

·ÌýÌýÌýÌýÌýÌý Maintaining high-quality records for patients within the serviceÌý

Detailed job description and main responsibilities

Workload at HMP Exeter

HMP Exeter receives approximately 190 receptions with 121 referrals to the mental health team per month. All referrals are triaged by the In-Reach Team. The in-reach teams hold a caseload and within this caseload, a number of patients would be subject to CPA management. New assessments requiring psychiatric input are slotted into the psychiatric clinic and then brought back to the team for discussion and planning their treatment and support. ÌýWhilst there is flexibility in allocation of individual responsibilities, the overarching model is one of integrated working across the mental health multidisciplinary teams.

There are no on-call commitments associated to this role.

This post-holder will have his or her base at HMP Exeter for the allocated PAs, where the mental health services comprise a full multidisciplinary team, including mental health practitioners, a Support Time and Recovery worker, Psychologists and Assistants, a Psychotherapist/Systemic practitioner all supported by an Admin Assistant and a Custodial Flow Co-ordinator, again all of whom work closely with all other healthcare colleagues. .
