Do you want to be part of a team making Hammersmith & Fulham a great place to grow old?Ìý The Community Independence Service (CIS) provides planned and unplanned nursing and therapy interventions for older residents who have had a recent deterioration in their physical health and is run jointly between West London NHS Trust and CNWL Foundation Trust across the three inner London Boroughs.Ìý We are looking to recruit an enthusiastic and self-motivated clinician, who will be an integral member of the team.

You will have excellent communication skills, experience completing person-centred holistic physical and cognitive assessments and care for people with moderate to severe frailty.Ìý You will delegate work to the team coordinators and be supported by local geriatric consultants.Ìý You will have good organisational skills and be able to manage your own workload to meet competing priorities. The role involves collaboration with internal and external stakeholders across the system.
Main duties of the job
Working as a member of the Community Independence Service you will supported by registered allied health clinicians and nursing staff you will to deliver skilled health and social care to people in their homes; the care you deliver will be based on delegated tasks by a senior clinician following thorough holistic assessment of person’s clinical and personal needs and their agreement of an individualised care plan.

The care you deliver will be within the boundaries of your role, focusing on supporting people with a range of complex needs and long term medical conditions (mental and physical). The aim is to reduce the impact of health inequalities, improve access to health and care services, address complex health, social and housing needs, reduce social isolation and optimise wellbeing.

Essential criteria
- Level 2/GCSE A-C (9-6) English language and Maths
- NVQ 3 or equivalent
- IT Skills
Desirable criteria
- NVQ assessor qualification
- Foundation degree/level 1 diploma modules or willingness to undertake
- Other equivalent qualifications and or evidence of further learning
Experience
Essential criteria
- Relevant Health or Social care assistant or demonstrate appropriate and relevant work experience
- Experience of autonomous working
Desirable criteria
- Experience of working within a community or multidisciplinary team
- Experience in Rapid Response/Acute/rehabilitation
- Admission/Venepuncture teams/Community Discharge teams
Knowledge
Essential criteria
- Ability to communicate in a variety of settings with patients of varying levels of understanding
- Able to present factual information and refer questions to others where appropriate
- Able to deal with sensitive issues with tact and diplomacy
- Basic working knowledge of IT and emails
- Ability to develop effective interpersonal relationships with colleagues in the healthcare setting
Skills
Essential criteria
- Carry out clinical interventions as delegated by a registered nurse or registered health care professional
- Ability to triage and carry out baseline assessments e.g. blood pressure, pulse, temperature, understanding of SBAR and deteriorating adult assessment
Desirable criteria
- Experience patient personal care, phlebotomy and wound care assessment and other advanced associate or technician tasks such as ECG, catheterization, bowel and bladder care.
