Job overview

To set and maintain standards of care and deploy, co-ordinate and supervise the nursing team and learners as required.Ìý

To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.Ìý

To be responsible for the team’s assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am-8pmÌý

To be responsible for the team’s maintenance of patient and other recordsÌý

To participate in the long term management of patients within the localityÌý

To have continued responsibility for a defined caseload.Ìý

Main duties of the job

To demonstrate discretion and respect when communicating with patients, colleagues and other agencies.Ìý

To use tact and diplomacy to provide and communicate complex, sensitive information where motivational negotiating empathetic and reassurance skills are needed, where barriers exist to understanding, maintaining confidentiality within Trust Policies.Ìý

To communicate effectively with all members of Primary Health Care teams. Develop networks for effective communication between health, statutory and voluntary services including patient forums.Ìý

To communicate and work together with other teams within Adult CommunityÌý

Services, and other providers, to ensure optimum patient centred care. Use various methods of communication including oral, written, electronic or other verbal or nonverbal methods.Ìý

To involve patients and carers in the planning of care programmes and encourage self-management where appropriate.Ìý

Essential criteria
  • Registered Nurse
  • Practice Supervisor/Assessor or equivalent
  • Specialist Nursing Practice(District Nursing) Degree or will undertake training
  • Nurse Prescribing or will undertake training
  • Knowledge of recent developments in NHS
  • Knowledge of Evidence based practice
  • Evidence of personal development
  • Knowledge of NMC code of professional conduct
Desirable criteria
  • First Level Degree
  • Independent Prescriber
  • Relevant courses in Long Term Conditions Management
  • Course of Top to Toe examination/ diagnosis Experience
