Job overview
We are looking for two enthusiastic and organised team coordinators with experience of working within an administration hub or similar environment to work in our Single Point of Access Service.Ìý
We are transforming our SPA service, which currently covers both Hounslow and Richmond Boroughs.Ìý The service will be separating into two single point of access services from April 2025, whereby these two posts will transfer to Kingston and Richmond NHS Foundation Trust to work in the Single Point of Access Service for Richmond Community Services.
ÌýThe administration hub operates between the hours of 07:00 – 19:00, 7 days a week, 365 days per year.Ìý Shifts will involve early, late and weekend working, these follow a pattern and are known in advance.
ÌýStrong computer skills, with experience in using Microsoft and associated packages and excellent communication, organisational and interpersonal skills are essential.
You will be able to work well using your own initiative and cope well under pressure. Good time management skills, flexibility and a positive professional approach are essential.
Main duties of the job
- Ability to work autonomously and as part of a Team.
- Provide an initial point of contact for referrers to for Hounslow and Richmond Community services including community nursing, end of life care (EOLC) and urgent response services.
- Support the staff on duty by completing an initial screening of calls to the Single Point of Access (SPA) providing signposting to other services if not within the services referral criteria.
- Liaise with Clinicians and communicate appropriate referrals to the adult clinical services including Night Nursing staff providing home visits overnight and urgent response services
- Work with other healthcare providers and agencies to support hospital admission avoidance plus timely and safe discharge of patients to the community setting.
- Input and upload referrals to System One, completing triage templates to ensure all relevant information is obtained and adding to the correct Caseload.
- Receive and provide handover with the overnight single point of access both administrative and clinical.
- Participate in the ongoing development of the service.
- Complete accurate patient records to ensure continuity of care.
