Job overview
We are looking for talented and dynamic physiotherapist to join the Hounslow Falls Prevention and Bone Health Service. ÌýThe service aims to enable people to work towards self-management and enable understanding of health and functional factors that could contribute to falls. You would be joining the team at an exciting time as we work in partnership with borough colleagues to enhance links between clinical and community services aimed at enabling the population of Hounslow to live well and reduce their risk of falls now and in the future.
You will be working as part of a multi-disciplinary team, including Occupational Therapy, Dietetics and Nursing, with patients and their carers to offer a multi factorial falls risk assessment followed by a range of multi-disciplinary interventions. ÌýYou will also work closely with colleagues in the acute Falls/ Frailty clinic, community Stay Steady and Active services and the local authority home safety team. ÌýYou will provide support, supervision and training for junior and support staff, and students.
Main duties of the job
The service is delivered through multiple site clinics and home visits. ÌýThe post holder will work flexibly and be mobile across the Hounslow Borough and in hospital settings. ÌýYou may be required to work some weekends to maximise patient access to the service.
• Ìý ÌýProvide holistic, multifactorial falls risk assessments and comprehensive fracture risk assessments and bone health management for clients who are at risk of, or worried about falling.
• Ìý ÌýTriage referrals received from GPs, other health professionals and others, being aware of quality & appropriateness of referrals, and deal with these as necessary.
• Ìý ÌýIndependently manage a caseload without direct supervision and to be professionally and legally accountable for all aspects of own work, including the management of clinical risk.
• Ìý ÌýProvide high quality comprehensive clinical interventions based in best practice.
• Ìý ÌýParticipate in pathway design and promotion of the service to encourage new sources and routes of referral to optimise the scope and impact of the service.
• Ìý ÌýAssist the team leader in the provision of the service as necessary.
• Ìý ÌýComply with the Chartered Society of Physiotherapy Core Standards of Practice and Health Professionals Council Standards of Performance Conduct and ethics of all other relevant national and local policies and procedures.

- Degree in Physiotherapy
- HCPC registration (if international to be sought/ in progress)
Experience
Essential criteria
- Relevant post graduate experience as a physiotherapist related to the role
- Ability to relate current experience and skills to community treatment
- Worked within a range of clinical settings as part of an MDT
Desirable criteria
- Worked within a community service as a physiotherapist
- Supervision of assistant staff or students
Skills and Abilities
Essential criteria
- To understand and apply professional standards, legislation and clinical guidance to practice
- To work as an autonomous practitioner; organising, prioritising and delegating work appropriately
- Professional communication skills including the ability to manage conflict or complex situations safely and effectively
