Job overview

  • To work as part of the Integrated Crisis Assessment Hub which provides intensive mentalÌýhealth support to people experiencing a mental health crisis. To provide support, give timeÌýand thus promote Service User recovery, to focus on the direct needs of Service Users, to work across boundaries of care and to work within the Care Programme Approach (CPA).

  • ÌýAssisting clinicians to assess, plan, implement and evaluate individual care plans.

The post holder will work with service users, individuals and families / carers and other stakeholders to ensure the persons needs are met, utilising where appropriate community facilities.

Main duties of the job

Key Responsibilities:

Ìý

1. To help ensure that service users engage effectively with the agreed Care Plan and access appropriate services

Ìý

2. To be responsible for liaising with and providing information to other agencies involved in the care of the service user.

Ìý

3. To develop a rapport and a therapeutic relationship with service users, within appropriate boundaries.

Ìý

4. To provide regular and practical support to service users and their carers (as appropriate) in developing and managing independence and maintaining dignity and self-respect

Ìý

5. Ensure that relatives and carers of service users, where appropriate, have a good understanding of the condition of the service user and feel able to communicate effectively with the clinical area to answer any queries or concerns

6. To provide a high-quality evidence-based and individualised mental health care to a clearly defined group of patients, relatives and carers in the community in which they live.

7. Assessing and using professional judgement to assess level of need and urgency, as well as obtaining further information as required, determining appropriate intervention and proposed outcomes of intervention

8. To use reasoning skills to assess how service users’ mental health difficulties impact upon their occupational areas and performance

9. Plan care in association with other members of the multi-disciplinary team and involve service user and where appropriate, carers in the care planning and care delivery, having explained options and obtained consent to care and treatment

10. Ensuring assessment, support/care-plans and intervention is shared and agreed with relevant individuals and agencies

11. Managing a small caseload of service users, offering a range of interventions, for a stated limited period

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust.Ìý All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. ÌýAs part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes.Ìý The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,551 to a maximum of £5,735 p.a. pro rata for part time).
