Our hospitals and community sites provide a wide range of inpatient and outpatient services, as well as emergency and planned care. The Trust is rated as 鈥済ood鈥 or 鈥渙utstanding鈥 in over three-quarters of the services inspected by the Care Quality Commission.
Every year our work includes performing 10,000 procedures in our theatres; bringing seven and a half thousand new lives into the world; carrying out 570,000 visits to patients in their homes or communities and providing emergency care for more than 300,000 patients arriving in our busy Urgent and Emergency Departments.
LGT is a centre for the education and training of medical students enrolled with King鈥檚 College London鈥檚 GKT School of Medical Education. We are a training centre for nurses, midwives and allied health professionals. Job overview
An exciting new opportunity has arisen for an Admiral Nurse to join our team at Lewisham & Greenwich Community Services. Admiral Nurses are specialist dementia nurses who work in the community to support patients and family/carers living with dementia. The successful candidate will be installing a brand new service bringing support and expertise to our patients and their carers in Lewisham Community.
The post holder will work collaboratively within our community team and the wider MDT and be supported by our experienced frailty team as well as being part of a national network of admiral nurses.听

Main duties of the job
In line with the Admiral Nurse Competency framework at an enhanced specialist level, the post holder will provide:
- Specialist bio-psychosocial assessment and support for people with dementia and families with complex needs
- Facilitation of education and training for staff in the delivery of evidence-based dementia care
- Clinical leadership with policy development and quality improvements in relation to dementia care
- Consultancy to other health and social care professionals regarding dementia

Detailed job description and main responsibilities
路 听 Provide clinical leadership on the care of people with dementia and their carers/families with complex care needs from peri diagnosis to post bereavement support
路听Provide specialist nursing assessment, using the Admiral Nurse Assessment Framework for carers/families affected by dementia
路听听听听听听 Provide a range of bio-psychosocial interventions to promote health and wellbeing for people with dementia and their carers/families
路听听听听听听 Work collaboratively with other professionals to improve the identification and management of co morbidities and frailty for people with dementia and their carers/families
路听听听听听听 Develop and support care plans for people with dementia and families including positive risk management听
路听听听听听听 Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia
路听听听听听听 Provide care coordination for a clinical caseload of families with complex needs as identified within triage assessment in clinics or via home visit if required
路听听听听听听 Ensure the safeguarding of vulnerable adults and that all legal requirements relating to Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007), and Deprivation of Liberty Safeguards (2009)[1] are considered and adhered to in relation to patient care
路听听听听听听 Act as an advocate for people with dementia and their carers/families to promote their rights and best interests
路听听听听听听 Participate and lead on case conferences, continuing health care processes and best interest meetings where required
路听听听听听听 Work collaboratively with health, social and voluntary sector services to achieve coordinated and integrated care
路听听听听听听 Work in partnership with colleagues to promote the use of advance care planning for people with dementia and to support and inform families, around advance care planning, best interest decisions and end of life care
路听听听听听听 Manage and advise on complex care transitions for people with dementia and their carers/families to reduce the frequency of crisis situations
路听听听听听听 Ensure that care is delivered in accordance with relevant local and national guidelines and all relevant Lewisham and Greenwich NHS Trust policies
路听听听听听听 Recognise the limits of own competency and professional boundaries and make appropriate and timely referrals to other services where required

Education /Qualifications/Training
Essential criteria
- Registered Nurse (RN/RNMH/RNLD)
- Post graduate qualification in relevant field
- Evidence of continued professional development/training in relevant areas, including dementia and leadership
- Commitment to continued professional and practice development through Dementia UK鈥檚 Admiral Nurse Academy
Desirable criteria
- Postgraduate qualification in dementia/leadership
Experience
Essential criteria
- Significant post registration clinical experience of supporting people with dementia and their carers/families
- Experience completing holistic nursing assessments and providing bio-psychosocial interventions
- Experience facilitating and delivering training/ development of staff
- Working as an autonomous practitioner
- Experience managing care transitions
- Experience of professional supervision of staff, including staff development, critical companionship, and role modelling
- Experience of audit/evaluation of clinical practice
- Experience of collaborative/multi-agency working
Desirable criteria
- Experience of leading/managing a service
