Community Nurse - Learning Disabilities (Complex Physical Health)

We have an exciting opportunity to recruit a learning disability nurse to work as a Community Nurse in Complex Physical Health within Bromley. You will be part of a small team of motivated nurses supporting people with their physical health.

This is a fixed term contract, finishing 16th December 2025 to cover maternity leave for 1 year.

This Community Nurse role will provide high quality health care to adults with learning disabilities to improve outcomes. You will work within a multi-disciplinary team and will enable people with learning disabilities to access primary and secondary care.Ìý You should be enthusiastic and show an understanding of the complex needs of people with learning disabilities.ÌýÌý Ìý

We offer a range of training and development opportunities both within Oxleas NHS Foundation Trust and externally. You will receive regular supervision and join our learning disability nursing council across three boroughs.

Main duties of the job

·ÌýÌýÌýÌýÌýÌý The post holder will work in the Bromley CTLD Complex Physical Health (CPH) of Learning Disability nursing team, focusing on the additional needs of people with complex physical health needs.

·ÌýÌýÌýÌýÌýÌý The post holder will be expected to manage a caseload of adults, working autonomously with a broad spectrum of physical health issues.Ìý

·ÌýÌýÌýÌýÌýÌý They will provide holistic, high calibre, evidence based, skilled nursing care, assessments, and interventions.

·ÌýÌýÌýÌýÌýÌý The post holder will be expected to facilitate a variety of health education and health promotion training and group work, with clients and carers as needed.

·ÌýÌýÌýÌýÌýÌý The post holder will support students and junior staff as required and will undertake to provide a rich learning experience for them.

·ÌýÌýÌýÌýÌýÌý The post holder will work collaboratively as part of a multi-disciplinary team and in partnership with other agencies/services.

·ÌýÌýÌýÌýÌýÌý The post holder will lead on providing robust health liaison to acute and primary care to ensure our service users are able to access mainstream health services.

·ÌýÌýÌýÌýÌýÌý The post holder will have a good awareness of the issues surrounding diversity and multi-cultural communities.

Detailed job description and main responsibilities

To apply national strategies, frameworks and plans to identify to meet the needs of service users in relation to their Physical Health and Wellbeing.Ìý

To identify and provide appropriate support to the social well-being of individual service users.Ìý

Work alongside the MDT and wider services to address issues to service users accessing services relevant to their individual health and wellbeing needs, as well as their social needs.Ìý

Provide health promotion relevant to the individual needs of the service user, and provide support to their support network to more widely promote and improve their health, and prevent avoidable hospital admissions where possible.Ìý

Liaise with mainstream services, and advocate the individual needs of the service users to promote their access to these services where required.Ìý

Develop own professional skills and knowledge by engaging in continued professional development that are both mandatory and relevant to the role.Ìý

To work in line with the NMC Code of Conduct at all times.Ìý

Essential criteria
  • Registered Learning Disability / Mental Health Nurse
Desirable criteria
  • Additional qualification relevant to the role being applied for

Experience

Essential criteria
  • At least 3 years post registration experience of working with people with learning disabilities
  • Experience of supervising junior staff and students
Desirable criteria
  • Experience of working in a community setting
  • Experience of working in a multi-disciplinary team

Skills / Abilities / Knowledge
