The role is for a bank band 5 medicine management technician. The hours are 9-5pm including late shifts and weekends as per rota.
Main duties of the job
The post holder will:
路聽 Have experience of working in a hospital pharmacy post qualification.
路聽 聽Provide a high quality patient focused medicine optimisation service, helping to provide a flexible and responsive service to patient needs.
路聽 聽Collaborate with the multidisciplinary team to deliver patient focused care to patients and ensure a timely supply of medicines, utilisation of patients鈥 own medicines and counselling on medication.
路聽 聽Participate in medicines management, ward top-up dispensary and ward-based dispensing.
Working for our organisation
Medicines Optimisation Service Provision
1.1 To be responsible for the provision of a high quality patient focused medicines聽optimisation service to patients of the Trust, associated hospitals and聽community clinics.
1.2 To facilitate the provision of medicines to patients to assist in their timely聽discharge.
1.3 To carry out audit and project work.
1.4 To check patients鈥 own medicines for suitability of reuse and highlight any聽shortfalls or queries with the patients prescribed/ not prescribed medications聽to the ward pharmacist.
1.5 To be responsible for gaining medication histories e.g. telephoning GPs when聽appropriate.
1.6 To support the dispensary in the timely dispensing of discharge medication.
1.9 To counsel patients as identified according to procedures and ensure that all聽discharge medicines are current.
1.11 To assist in the training of pharmacists, pharmacy technicians and nurses in聽the one-stop dispensing and self-administration system.
Detailed job description and main responsibilities
2. Discharge Service Provision
2.1 To be responsible for the provision of a high quality patient focused discharge聽service to patients of the Trust and help ensure that service provision is聽flexible and responsive to patient needs.
2.2 To provide advice on the technical aspects of the service.
2.3 To facilitate the provision of medicines to patients, to assist in their timely聽discharge.
2.4 To check patients鈥 own medicines and those dispensed ready for discharge for聽their suitability for supply at discharge and highlight any shortfalls or queries聽with the patients prescribed/ not prescribed medications and the discharge聽prescription to the ward pharmacist.
2.6 To support the dispensary in the timely dispensing and supply of discharge medication.
2.9 To counsel patients as identified according to procedures and ensure that all聽discharge medicines are current.
3. General Service Provision
3.1 To provide a high quality, patient focused pharmaceutical service to inpatients,聽out-patients, discharge patients, community clinics, and outlying聽nursing homes if needed.
3.2 To work in the dispensary to assist in the medicines optimisation process and聽as part of the weekend/late duty rota.
3.3 To work as an ACT in the dispensary, and if required at a ward level.
3.4 To demonstrate patient focused approach including optimisation of patients鈥 knowledge of their medicines.
3.7 To communicate with patients and health care professionals.
3.8 To input prescription information in the pharmacy computer system for dispensing and supply of medicines to patients.
3.9 To collect and handle prescription monies according to Government and Trust聽policy.
3.11 To have a working knowledge of pharmacy stock control and ordering systems.
3.12 To have a working knowledge of all procedures, legislation or national聽guidelines relating to the storage and dispensing of unlicensed medicinal聽products, controlled medicines, oral chemotherapy and clinical trials.
3.14 To be working as an ACPT (accredited pharmacy checking technician) adhering to the minimum 8 hours per month.
When practicing as an ACPT, to work within defined boundaries as set out in聽the Pharmacy Standard Operating Procedures and Medicines Policies.
It is highly recommended for ACPT to have professional indemnity cover.
4. Education and Training
4.4 To participate in education and training of other healthcare professionals.
4.5 To participate in education and training schemes and meetings organised聽within the department.
General Responsibilities
6.1 To participate in all relevant departmental meetings and contribute to
effective communication within the department.
6.3 To undertake any other duties commensurate with the grade as requested by聽the Chief Pharmacist.
6.4 To be aware of and apply, relevant legislation such as the Health and Safety at
Work Act, Control of Substances Hazardous to Health, GMP and Medicines鈥櫬燗ct.
6.5 To at all times practice in accordance with the Code of Ethics of the General聽Pharmaceutical Council.
6.6 To uphold the security of medicines and the premises at all times.
6.8 To take part in the weekend and bank holiday service according to rota.
Safeguarding Children and Vulnerable Adults At Risk

IT Skills -聽All staff are required to demonstrate a level of IT literacy skills appropriate to their job, as it is fundamental in delivering good quality efficient health care.
Have good IT knowledge and training experience on Pharmacy systems
- 聽Provide dispensary-based patient services to inpatients and outpatients
- To support all pharmacists (including pre-registration trainees) in the provision of the service, and if qualified to do so, practice as an聽聽聽 ACPT.
- To help audit the service against set standards and assist in the review of service provision.
- To ensure the safe use of patients鈥 own medicines to optimise cost efficiency.
- To ensure the safe storage of medications stored in patients鈥 lockable cabinets.
- To participate in financial evaluation of medicines and expenditure reporting.
- To provide specialist pharmacy technical input into the electronic prescribing and medicines administration system (ePMA), contributing towards both operational and strategic development聽聽Through provision of an effective Pharmacy service, this post will;
