Job overview
An exciting opportunity has arisen to join our dedicated community heart failure team in Brent as a Band 7 community heart failure specialist nurse on a part-time basis.
In this crucial role, the Heart Failure Specialist Nurse will work as an autonomous clinical practitioner, providing a high-quality community heart failure service to a specific caseload of patients, enhancing their quality of life, promoting health excellence and independence within the community, preventing unnecessary hospital admissions, and providing education and health promotion.

The successful candidate will be expected and supported to complete the physical assessment of the adult and non-medical prescribing qualifications.
You will be an experienced cardiology nurse with relevant post-registration training.Ìý
You will have supervisory responsibility and be able to guide and teach members of the clinical team.

Main duties of the job
A flexible approach to practice, excellent communication skills, the ability to work autonomously and a qualified driver are all essential for this post.
The service provides complex care to patients with heart failure within the community setting.
The post-holder will be joining a multi-disciplinary team working with other Heart Failure Specialist Nurses, Cardiologists, and the wider Brent Integrated service.
The position involves working closely with the primary and secondary care teams, to provide specialist heart failure care and expertise to patients diagnosed with heart failure, and to ensure that patients receive seamless care in hospital, at home and in the outpatient setting. Ìý
As well as the clinical delivery, you will be required to lead on various service development projects such as audits, patient stories, and patient engagement. In addition, you will be contributing to up-skilling other health care professionals to provide a high standard of care.Ìý

Education/Qualification
Essential criteria
- Registered Nurse
- Current active registration with NMC
- Degree or relevant post registration experience
- Qualification in independent nurse prescribing or willing to undertake qualification
- Hold or working towards qualification in physical assessment
Desirable criteria
- Chronic disease management course/ experience.
- Heart Failure/Cardiology Course
- Counselling skills/ course.
Experience

- Ability to develop new initiatives

- Community experience
- Experience of working with Heart Failure patients
- Experience in conducting Audits
Desirable criteria
- Experience of liaising with external stakeholders
Skills and Knowledge
Essential criteria
- Ability to interpret investigative results i.e. blood results, ECHO.
- Sound knowledge of national guidelines and standards on cardiac /heart failure care.
- A working knowledge of clinical governance and evidence-based practice.
- Ability to make decisions and priorities actions
- Ability to be proactive and resourceful
- Excellent documentation skills
- Ability to demonstrate an understanding of the holistic needs of the client group.

- Experience of working within protocols
- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
