Job overview
The post holder will provide a comprehensive service to the Trust with a focus to facilitate effective and timely discharge of patients no longer requiring acute care.
The role requires clear communication between the Hospital Discharge Team, the Discharge to Assess Hub and the wards to ensure safe timely discharge of all patients .

- To support the Discharge Team in facilitating timely discharges for complex patients
- Attend Whiteboard rounds and ward rounds, identify actions required and actively manage agreed discharge plans for patients
- Assist in the completion of discharge check lists to ensure accurate discharge planning is evident, under the direction and supervision of the registered nursing staff
- Transfer patients to the discharge lounge if required

Detailed job description and main responsibilities
Main Duties and Responsibilities
- To support the Discharge Team in facilitating timely discharges for complex patients
- Support the maintenance of Advantis ward to ensure it reflects the current discharge plans .
- Attend Whiteboard rounds and ward rounds, identify actions required and actively manage agreed discharge plans for patients with predicted discharge dates within the next 3 days
- Maintain whiteboards/Advantis ward information to ensure it is up to date
- Assist in the completion of discharge check lists to ensure accurate discharge planning is evident, under the direction and supervision of the registered nursing staff
- Transfer patients to the discharge lounge if required
- Liaise with the Registered Nurse in charge /Ìý shift coordinator and resolve issues preventing prompt patient transfer
- Under the direction of the Registered Nurse only, ensure plans are agreed with patient and carers for discharge and ensure they are fully aware of any requirements to assist discharge
- Utilise IT systems to secure and record timely and accurate data relevant to the role and function

If successful this vacancy will require a Standard DBS check at a cost of £26.50. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
