The Aintree Frailty Service is expanding! And we are looking to recruit someone who is passionate about the care of older patients to join our team.

The Aintree Frailty Service delivers front-door assessment to patients over the age of 65, presenting with frailty symptoms. Each patient receives a Comprehensive Geriatric Assessment delivered by a multidisciplinary team, including pharmacy.ÌýÌý

LUHFT employs over 120 pharmacists across 3 sites providing clinical pharmacy services to all major specialties. This post is based on the Aintree site of the Trust.Ìý

We are looking for a dynamic, motivated and forward thinking clinical pharmacist to work as a team and help lead our established clinical pharmacy team within the Department of Medicine for Older People.Ìý

We have a fully integrated electronic prescribing system along with unique in-house systems to help you deliver excellent clinical care to your patients. The successful candidate will be responsible for providing leadership and delivery of a high quality, proactive and patient focused clinical pharmacy service.

Main duties of the job

Assist the Lead Divisional Pharmacist- MedicineÌý in the development of pharmacy services and medicines management within the Frailty Service and Department of Medicine for Older People (DMOP).

To provide a safe and effective advanced clinical pharmacy service to theÌý Frailty Service.

Assist the Lead Divisional Pharmacist-Medicine in providing clinical training and supervision, relevant to theÌý Frailty Service.

Provide non-medical prescribing services to the Frailty Service.

To work with the current Advanced Specialist Pharmacist in Frailty and DMOP Services in delivering the key responsibilities.

Detailed job description and main responsibilities

To provide an advanced clinical service to theÌý Frailty Service within the Division of Medicine in accordance with professional, departmental and Trust policies.

Provide highly specialist pharmaceutical information to medical and nursing staff and patients within the Directorate and to General Practitioners and other staff within the Trust where appropriate.

To act as an independent prescriber in accordance with the Trust's Non-Medical Prescribers policies and guidelines (once registered as a non-medical prescriber)

Participate in Consultant ward rounds and multidisciplinary meetings relevant to the designated wards and or directorate,

Essential criteria
  • Excellent communication skills, both verbal and written
  • Ability to work on own initiative and as part of a team.
  • Self-motivating and able to motivate/inspire others
Desirable criteria
  • Ability to affect and manage change

Qualifications

Essential criteria
  • Vocational Masters degree in pharmacy.
  • Registration with the General Pharmaceutical Council (GPhC).
  • Diploma in Clinical Pharmacy or equivalent documented experience
  • NMP Qualification (or registered as a NMP within 3 months with the GPHC)
Desirable criteria
  • MRPharmS
  • UKCPA Member

Experience

Essential criteria
  • Significant experience in a relevant specialist pharmacist role
  • Antimicrobial stewardship experience
  • Demonstrable commitment to continuing professional development / education and training.
Desirable criteria
  • Experience as a NMP

Knowledge

Essential criteria
  • Excellent clinical knowledge and practical clinical skills for the relevant specialist role
  • Good all round clinical knowledge and practical clinical skills

Other

  • Computer literate
