An exciting opportunity has arisen for a highly motivated forward thinking advanced clinical pharmacist to join us at LUHFT.Ìý

ÌýSupporting the delivery and development of the medicines Optimisation agenda across LUHFT and Cheshire & Mersey Integrated Care Board (CMICB), working with healthcare professionals from all disciplines.Ìý

The post will create an opportunity for an enthusiastic pharmacist to learn about medicines optimisation with our team of experts and help create support forÌý medicines optimisation activity across the ICB and within the Trust.Ìý

Main duties of the job

  • Assist theÌý Lead Pharmacists for the delivery of medicines optimisation activity to pharmacy services and medicines management within the directorate.
  • ÌýCo-ordinate the agenda and ongoing actions of LUHFT Medicines Clinical Effectiveness Group (MCEG)
  • ÌýCo-ordinate and contribute to the review and approval of new medicines requests to LUHFT MCEG
  • ÌýCo-ordinate and contribute to the review and submission of individual funding requests to the relevant NHS commissioners
  • Review and advise on medicines-related documents planned for submission to LUHFT MCEG in collaboration with clinical pharmacists and LUHFT Medicines Information team.
  • Contribute to the development and update of documents for inclusion in the CMAPG Formulary.
  • Deputise for the Lead Pharmacist, Medicines Optimisation at Trust and regional meetings/ committees and other meetings as necessary.
  • Provide a specialist clinical service to a designated ward in accordance with professional, departmental and Trust policies (See JD for further information)

Detailed job description and main responsibilities

  • ÌýCo-ordinate and contribute to the review and submission of individual funding requests to the relevant NHS commissioners
  • Review and advise on medicines-related documents planned for submission to LUHFT MCEG in collaboration with clinical pharmacists and LUHFT Medicines Information team.

  • Identify the clinical and economic impact of medicines use, develop and implement clinical guidelines, monitor drug expenditure and support the area to meet their clinical and financial targets.

Person specification

Experience

Essential criteria
  • Relevant post diploma clinical experience at specialist clinical pharmacist level
  • Experience of undertaking audit
  • Experience of clinical document development and review
Desirable criteria
  • Experience of preparing meeting agendas and taking responsibility for action logs

Qualifications

Essential criteria
  • Vocational masters degree in pharmacy
  • Membership of The General Pharmaceutical Council
  • Diploma in clinical pharmacy or equivalent documented clinical experience

  • MRPharmS
