Job overview

An exciting opportunity has arisen for a highly motivated, experienced and innovative occupational therapist to join the South Monmouthshire integrated service with the focus on providing rehabilitation in our purpose built step up/down unit based within the brand new purpose built Severn View care home. The step up/down service provides a residential setting for those people in need of continued rehabilitation to facilitate discharge from hospital but are not yet ready to return home or those who cannot manage at home due to a fall, illness, or crisis where hospital admission is not required. Working within a multi-professional team, the post holder will be expected to support the development and implementation of rehabilitation programmes tailored to the persons occupational performance needs to support the individual to return home.

Main duties of the job

The postholder will work alongside a band 4 OT technician, Severn View Park staff, reablement and the wider MDT in Severn View Park residential home to deliver rehabilitation to people to enable them to live as independently as possible and to return to their own homes.

The postholder will develop individual rehabilitation programmes to support the person to maximise their independence and occupational performance.

The postholder will support the delivery of reablement training to staff to ensure a rehabilitative approach during the persons stay to maximise independence

This role provides an excellent opportunity to develop caseload management skills. The successful candidate will need to be flexible, highly motivated, innovative and enthusiastic with the ability to deliver a high-quality individualised service with in-reach into other aspects of the pathway as required.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  • Degree or Postgraduate Diploma in Occupational Therapy
  • HCPC Registration Up to date professional port folio
  • Some specific training related to clinical area e.g. manual handling, seating and positioning, advanced communication skills, cognitive assessment, motivational interviewing.
Desirable criteria
  • Post-graduate experience of working with people with a range of conditions
  • Supervision experience of junior staff and/or students
  • Experience of working autonomously and setting own priorities
