Job overview

The post holder is required to support the continued development of high-quality clinical services to meet the holistic needs of patients living with dementia and lead the development of an organisational culture that enable equal and accessible healthcare service for this patient group.

Provide professional nursing leadership and expert consultancy on all aspects of clinical dementia nursing practice for the care of people with dementia and their families.

Be responsible for driving improvement of the quality of care received by persons living with dementia within the acute hospital setting.

Be accessible and have a visible presence, providing a responsive service advising and supporting ward and departmental staff, patients, and their families to ensure effective patient care across the whole patient pathway.

Work in an integrated and aligned manner with all relevant local and national work streams and programmes, leading on developments and innovation in dementia care and practice standards

Main duties of the job

Work autonomously and collaboratively with all members of the Corporate Nursing team, Safeguarding Team, Medical and Surgical Teams, Multi-agency and community Partners, Nursing, Midwifery and Allied Health Professionals.

Be an expert practitioner in all aspects of dementia nursing care and ensure the provision of specialist education and training.

Lead on developing clinical and practice standards for dementia using Clinical Governance framework, patient feedback, and audits.

Lead on the development and operational delivery of the Trust Dementia Strategy with support from the Deputy Director of Nursing

Maintain Clinical Competence and Credibility.

Provide strong professional clinical and managerial leadership to nursing and other members of the multidisciplinary team and act as a professional role model.

Act as a clinical expert to all areas providing care to people living with a diagnosis of Dementia.

Through effective leadership and role modelling, support ward staff in delivering high quality clinical care, where compassion is embedded to achieve the best outcome for the patient and a ensure positive patient experience.

In collaboration with the senior team, continually improve and maintain standards of fundamental patient care to meet the required local and national standards

Detailed job description and main responsibilities

Lead on the operational development and delivery of the Trust Dementia Strategy with support from the Associate Director Safeguarding and the Deputy Chief Nurse.

Ensure active involvement in The National Audit of Dementia (NAD) clinical audit program commissioned by the Healthcare Quality Improvement Partnership and lead on this for the Trust.

Take responsibility to ensure that the highest standards of professional and quality care are delivered within the trust policies, protocols, and guidelines.

Work across professional boundaries; act as a professional role model to Senior Sisters/Charge Nurse, nursing/AHP staff and other members of the multidisciplinary team.

In collaboration with members of the multidisciplinary team, ensure the users of the service and their families have an opportunity to feedback (via Surveys, Family and Friends Test, concerns, and complaints). Work alongside managers/leaders to support the evaluation of feedback and redesign services as required. Including ensure service development and evaluation is co-produced.

Work collaboratively across organizational boundaries with partner organizations, with relevant colleagues and clinical experts in other organizations, to review and share quality assurance and improvement strategies.

Proactively and positively contribute to the successful overall performance of the Trust.

To provide clear, visible leadership to create a culture of compassion, creativity and candour which is truly committed to improving the patient experience at every step of their care pathway and support the inclusion of patients and carers in the co-design of services

Provide strong, professional, and visible leadership to staff within the clinical areas. Lead and motivate staff by giving clear direction and setting achievable objectives.

To provide assurance to the board, via the dementia steering group, including verbal and written reports to corporate steering groups and Divisional Governance forums on themes relating to Dementia.

Lead on the development, delivery, monitoring and evaluation of relevant specialist training and education, including developing nursing and multi-professional staff knowledge and skills to enhance the quality experienced by the service user and their families.
Be responsible and support the work and development of the dementia volunteers and support the work of the enhanced care team.

Participate and take an active role within corporate training opportunities.

Access opportunities to support own personal & professional development.

Communicating, influencing, and working with the multidisciplinary team in different ways to improve quality of care, and inform clinical decisions. Promote innovation and participate in benchmarking and quality audits to measure effectiveness of current practices.

Support teams to implement evidence-based practice, initiate, manage, and evaluate changes that lead to improved, high standards of care and in particular any service changes.

To advise on issues relating to complex discharges for patients with dementia.

Identify complex problems in order of importance and develop actions to address these making decisions and steps to ensure appropriate actions are completed.

To promote the health and well-being of staff, patients, and their carers.

To ensure the development of proactive and effective communication and relationships with internal and external stakeholders.
Be an active member of the clinical nursing team contributing to Trust wide nursing groups and meetings.

Education, Training and Qualifications

Essential criteria
  • Registered Nurse
  • Educated to Masters level or equivalent level of education, training or experience
  • Evidence of Post registration and continuing professional development and learning
  • Teaching Qualification or relevant qualification /experience

Knowledge and Experience

Essential criteria
  • Substantial experience of care management, experience of working in a senior role
  • Experience of Dementia Care
  • In-depth Knowledge and understanding of Mental Capacity Act, Best Interest and Deprivation of Liberty Safeguards.
  • Experience of Interagency working and professional challenge
  • Evidence of developing policy, guidelines and managing resources
  • Demonstrate extensive relevant clinical experience of working in an acute hospital setting at a senior level
  • Understanding of health policy and the national and local nursing agenda
Desirable criteria
