An exciting opportunity has arisen for a Discharge Facilitator to work on Gastroenterology/HepatologyÌý for a 6 month period to support the safe and timely discharges for our patients.Ìý

Ward 230 is a 36 bedded , diverse speciality both acute and fast paced, with a forever evolving environment.

You must have excellent communication skills as liaising with all members of the Multi-disciplinary team, nursing and medical colleagues and patients and their relatives is a key aspect of the role.Ìý You must have excellent computer and literacy skills.

Main duties of the job

In summary the role is

  • To assist qualified nurses to plan and facilitate a timely discharge process for patients from point of admission to the ward environment to the discharge of patients to their own home or alternative care facilities
  • To ensure that the Bed Management Tool is accurately
