Detailed job description and main responsibilities

The department of Medicine is run on an integrated acute medicine and care of the elderly ward-based system, with most wards having at least three consultants, two Specialty Registrars, three Specialty Registrars/F2 trainees and three F1 trainees. F1 trainees are regularly rostered to clerking shifts on AAU under the supervision of the physician of the day. In the evenings and at weekends medical wards are covered by two F1 trainees, with support from the rest of the on-call team. At night the hospital is covered by a hospital at night team run by the specialist registrar and including F2 trainees and enhanced trained nurses. F1 trainees no longer work at night within the hospital. Formal handover to the hospital at night-time is an important component of the post for the on call F1 trainees.

Each ward has their own ward-based team of juniors including combinations of FY1, FY2, ST1 and ST2 trainees. Each ward also has ward based StR(s) and a combination of medical and COE consultants. The division runs a ‘physician of the day’ system, to cover the acute on call and provide a daily post take ward round of those admissions not retained in AAU or the short stay facilities.

Each ward operates a daily consultant multidisciplinary red to green board round to facilitate early discharge and to ensure all patients have daily consultant input.

Our Acute Admissions Unit (AAU) is in operation and comprises of short stay trolleys and a dedicated Same Day Emergency Care (SDEC) area.

  • Evidence that career progression is consistent with personal circumstances.
  • Evidence that present achievement and performance is commensurate with totality of period of training.
  • At least 24 months’ experience in medicine or an ACCS programme or equivalent (not including Foundation modules) by the time of commencement of ST3 training.

Eligibility

Essential criteria
  • Hold full registration and licence to practise with the GMC.
  • Previously worked in the NHS for a minimum of 6 months or an NHS clinical attachment.
  • Evidence of achievement of Foundation competences from a UKFPO affiliated Foundation Programme or equivalent by time of appointment in line with GMC standards/ Good Medical Practice including: • good clinical care • maintaining good medical practice • good relationships and communication with patients • good working relationships with colleagues • good teaching and training • professional behaviour and probity • delivery of good acute clinical care
Desirable criteria
  • Evidence of completion of CT/ST1 or equivalent competences in medicine at time of application, and CT/ST2 competences in medicine by time of appointment, supported by evidence from work-based assessments of clinical performance and Multisource Feedback (DOPS, Mini-CEX, CBD, ACAT) or equivalent.
  • Previous clinical experience in Elderly Medicine

  • Is up to date and fit to practise safely.

Language skills

  • All applicants to have demonstrable skills in written and spoken English that are adequate to enable effective communication about medical topics with patients and colleagues which could be demonstrated by of the following: PLAB 2 If applicants believe that they have adequate communication skills, but do not fit into one of the examples they need to provide evidence.

  • Competence in the immediate management of medical emergencies
  • Appropriate knowledge base, and ability to apply sound clinical judgement to problems.
  • Proficiency in a range of medical procedures

  • Able to maximise safety and minimise risk.
  • Attendance at relevant courses, e.g. ALS, ATLS, EPLS, APLS or equivalent
Desirable criteria
  • Personal attributes: shows aptitude for practical skills, e.g. manual dexterity.
  • Experience of managing common presentations of frailty

Academic/ Research skills

Essential criteria
  • Demonstrates understanding of research, including awareness of ethical issues.
  • Demonstrates understanding of the basic principles of audit, clinical risk management, evidence-based practice, patient safety, and clinical quality improvement initiatives.
  • Demonstrates knowledge of evidence- informed practice
Desirable criteria
  • Evidence of relevant academic and research achievements, e.g. degrees, prizes, awards, distinctions, publications, presentations, other achievements.
