Job overview
Surrey Downs Health & CareÌý are looking for a Band 7 Physiotherapist o be part of theÌý SDHC Home First Epsom UCR team. Community experience, experience in ED & rapid responsesÌý are required for this post and it is a short term bank position over Winter however you will be onboarded to Bank so will be able to pick up further shifts following this line of work. Home visits for new rapids & follow ups are key duties.
Hours are 34 hours per week in a working pattern of x3 12 hour shifts of 8am-8pm with a 40 minute break.Ìý

Main duties of the job
To undertake specialist physiotherapy and holistic assessment of patients as an autonomous practitioner, interpret results appropriately, use clinical reasoning skills and knowledge of evidence based practice, in relation to the individual’s clinical and psycho-social issues to determine treatment options, recommending the best course of action.
• To undertake treatment planning and implementation of agreed goal orientated intervention with client/carers.
• To contribute towards single assessment and trusted assessment processes
• To provide comprehensive intervention to patients making use of assessment and treatment facilities where appropriate.
• To be responsible for a clinical caseload working within multi-disciplinary teams, sharing care appropriately ensuring patients and carers receive optimum level of therapy and holistic care within available resources.
• To carry out multi-disciplinary screening assessments and discuss outcomes with multidisciplinary team members.

To lead and manage a community multi-disciplinary rapid response and intermediate care team, taking overall responsibility for the assessment, management and delivery of community rehabilitation, rapid response and intermediate care to patients in a variety of community settings and to be responsible for the therapy input to Home First Service patients. To promote effective teamwork within the wider Home First Service team, delegating activities appropriately, to the benefit of the patient and service delivery.

Essential criteria
- • AHP with appropriate Diploma/ degree • Current registration with HCPC • Further education in a relevant field to post gradual diploma or equivalent level, combination of education, further training and experience. • Evidence of CPD relevant to treatment of older adults, physical disability and rehabilitation. • Evidence of an up to date CPD portfolio
Desirable criteria
- • Qualification in team / service management or prepared to work towards. • Masters level study related to rehabilitation or working towards this.
Experience
Essential criteria
- • Evidence of recent post-graduate experience in core areas including elderly care, respiratory and rapid response. • Experience of working in a community/ primary care setting • Experience of negotiating and liaising within a multi-disciplinary/ interdisciplinary environment • Experience of working and assessing people with complex needs and delivering complex care plans • Experience of co-ordinating and managing a team • Experience of supervision and teaching others • Active participation in in service training. • . Experience in service development

- • Experience In multi-agency working • Experience in promoting services within a locality • Experience of working with clients requiring intermediate care. • Experience of working with in a virtual ward model in a community setting
Skills and Knowledge
