Job overview

We are looking for a highly dedicated Nurse, Occupational Therapist or Social Worker to join our Liaison Psychiatry Service at Epsom General听 Hospital in Epsom, Surrey.

Our team is an assessment and signposting service in the general hospital setting. and we also offer teaching and education about mental health to raise awareness. our team is a small multi disciplinary team consisting of clinicians, a service manager, a team administrator and a team consultant. Our service is a specialist service within Hospitals and Specialist services, we are a core 24/7 days service and our clinical staff works on a 27/7 roster covering long days, nights and twilight shifts.

We also offer opportunities for development and growth . You will need the ability to work autonomously whilst still contributing to the team as a whole. Previous experience of working in Liaison Psychiatry / HTT/community and across ages is desirable, although not essential. We offer excellent staff support through clinical supervision and teamwork.

You will have an excellent and positive approach to work, with an outstanding, demonstrable background of providing high quality care. You will have an insight into liaison psychiatry and the national guidelines impacting these services.

Main duties of the job

To carry out psychosocial assessments of people referred to the Liaison service from a variety of sources within the General Hospital, including A&E and Medical Wards.

To work with people presenting with a range of psychosocial needs arising in the context of physical illness, as well as people presenting to the hospital following self-harm or in a mental health crisis

To provide rapid psychiatric assessment and safe discharge planning in Emergency Department.

To agree on a collaborative plan of care with people who use our services and their carers where appropriate and arrange appropriate after-care

To ensure that the relatives, carers and friends are actively encouraged to participate in care, provided with information, help and support where necessary

To mentor, supervise and teach qualified and unqualified staff including students

To lead the care for a specified group of service users such as frequent AE attendees

To assist in the development and maintenance of good working relationships with general hospital staff, local mental health services, primary care and non - statutory organisations

Detailed job description and main responsibilities

To co-ordinate and monitor the delivery of person centered healthcare to a designated group of people who use our services, using complex clinical decision making where appropriate.

To be responsible for the full assessment of the psychiatric/psychosocial needs of those patients who have been referred to the service; including the comprehensive assessment of risks to self and others

To be responsible for the development, implementation and evaluation of robust discharge plans based on a clear assessment and formulation of the person鈥檚 needs; which may include urgent referral to secondary mental health services, social and voluntary services

To work as a member of the MDT in assessing and planning care and responding to changes in people who use our services To act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs, actively supporting the individual to fulfil these.To ensure provision of therapeutic activities, emotional support and stability to clients to ensure effective engagement whilst maintaining professional boundaries.

To create, develop and maintain professional supportive relationships with all members of staff, with other professionals and agencies to enhance recovery.

To act as role model in providing a service for people with mental health problems within the general hospital setting

Qualifications

Essential criteria
  • Professional Qualification as RMN/RN (Mental Health) or Occupational Therapist or Social Worker with professional registration (NMC/HCPC)
  • Willingness to complete Mentorship course

Experience

Essential criteria
  • Experience post qualification working with people with mental health problems
  • Independent assessment skills i.e. CMHRS duty, HTT assessment, liaison assessment or similar would be desirable
  • Awareness of personal and professional boundaries
  • Evidence of CPD
