An exciting opportunity has arisen to join the Trust as the Macmillan Palliative and End of Life Care Team Leader in Wexham Park Hospital. This postÌý encompasses a combination of clinical, managerial and leadership responsibilities.Ìý This is a permanent post co-ordinating and delivering the vision of the 7 Day Hospital Palliative Care service.Ìý
The successful applicant will work as part of a well-established Palliative Care and EoLC multi-disciplinary team who provide specialist advice and support to patients, their relatives and members of the health care teams within the hospital and community. ÌýÌý

Applications are invited from a Nursing & MidwiferyÌý professional background who have worked at a senior level in palliative care. ÌýYou should also have a Degree in Nursing, a recognised palliative care qualification, advanced physical assessment skills, be a non medical prescriberÌý with excellent communication skills.ÌýÌý
The postÌýholder will be proactive in leading service redesign in palliative and end of life care within the hospital in order to make services responsive to changing Ìýneeds.Ìý You will work closely with the Trust Lead for EOLC &Ìý and the Palliative Care MDT.
Main duties of the job
Provide leadership and support for the Palliative and End of life Care teamÌý to the Wexham Palliative care team.
Act in a consultancy role as an advanced clinical practitioner in palliative care, offering specialist advice, guidance and supervision to clinical staff, patients and carers
The post holder will act as an expert in palliative and end of care and advise on care for patients with advanced malignant and non malignant disease from time of diagnosis and throughout the trajectory of their illnessÌý
Lead developments in practice consistent with the patient care priorities set out nationally and locally. ÌýDevelop new roles and redesign services in line with best practice and evidence based care.

Detailed job description and main responsibilities
The post holder will lead and manage the palliative and end of life care team on a daily basis, ensuring that the team provides an excellent service to the Trust through evidence based practice, education, policy and guideline development.
Lead clinical care by providing expert assessment, planning, evaluation and support skills in aspects of palliative and end of life care.
The post holder should have advanced physical assessment skills and be a non medical prescriber as well as working towards a Masters degree/Advanced Clinical Practitioner
Maintain a high profile in clinical practice, promoting clinical excellence, initiating learning opportunities and promoting an effective learning environment

Qualifications
Essential criteria
- Degree in Nursing
- Masters degree or commitment to work towards completion
- Qualification in Palliative Care/Oncology/Frailty
- Non Medical Prescribing qualification
- Advanced Physical Assessment Skills qualification
- Leadership qualification
- Registered Nurse with current NMC registration
Desirable criteria
- Counselling qualification
- Teaching & Assessment qualification
Experience
Essential criteria
- Recent experience in managing and leading a team
- Ability to work at pace, establish effective working relationships and develop strong team working
- Ability to manage multiple and conflicting priorities and meet specific deadlines
- Proven leadership and management skills
- Experience of leading patient involvement/patient feedback initiatives
Desirable criteria
- Involvement in clinical audit and/or research
- Report writing
Knowledge
Essential criteria
- Ability to organise and prioritise own workload
- Knowledge and experience of management models, implementing change and/or new clinical practices
- Knowledge of clinical governance and risk management
Skills and Aptitude

- Able to lead, empower others and influence innovative practice and service development
- IT literate with good understanding of common packages
