Job overview

There is an exciting opportunity within Swansea Bay Virtual Ward Team for the right candidates.

We are looking for experienced and enthusiastic HCSW's with a Level 4 vocational qualification in a relevant Health Care Associated subject/ Certificate in HCSW Education.

We have a Band 4 Assistant Nursing Practitioner position positioned in the Upper Valley cluster.

These are new positions and the successful candidates will have access to ongoing support and a structured, robust induction and training package in order to develop into role.

Applicants to have excellent communication skills and have a flexible and adaptable approach to Community working

Main duties of the job

The post holders will be required to deliver high quality, compassionate, evidence-based care to patients at their home following a recent hospital admission or GP referral, utilising person-centred approaches which promote health and independence.

Post holders will carry out a range of clinical skills appropriate to their area of work, in line with their scope of practice and the all-Wales Band 4 Assistant Practitioner (Nursing) Governance Framework.
