Job overview

The Integrated Care Home Lead is responsible for supporting the assessment and management of the health needs of care home patients across Coventry and for developing and implementing care programmes, including those for care home residents with complex frailty needs, end-of-life care, and patients identified as at risk of hospital admission. Supporting the implementation and ongoing monitoring of virtual care strategies. Integrated Care Home Lead will clinically supervise and provide support to a team of staff.  These posts are locality-based and you will be based at either Willenhall, Tile Hill or Newfield House.

Main duties of the job

  • Take responsibility for the complex and holistic assessment or delegation of planning, implementation, and evaluation of care on an individual basis, utilising recognised models and tools, e.g., end-of-life care and long-term conditions.
  • As an Integrated Care Home Lead there will be a need for a varied clinical skill set, including, but not limited to:

oÌýÌý End of Life Care- Palliative care 

oÌýÌý Patient education and training of self-management techniques

oÌýÌý Long Term Condition Management – Heart Failure, Respiratory, Frailty, Dementia

oÌýÌý Social prescribing

oÌýÌý Recognising health care needs changing- ability to refer to specialist services- Dietitian, District Nurse, Speech and Language, GP, hospital services, Mental Health Services.

  • Named responsibility for the management of a defined caseload.
  • Acts as patient’s advocate where appropriate.
  • Be proactive in offering health promotion and participate in health promotion activities for a defined caseload.
  • Utilise health needs assessment for the local population.
  • Ensuring excellent working relationships and communication links with all stakeholders including acute hospital staff, Local Authorities, Social Care teams and voluntary services as appropriate for patient care.
  • Provide the care staff with training and education.

Communication

·ÌýÌýÌýÌýÌýÌýÌýÌý Demonstrate excellent communication and negotiation skills in situations which may be complex, highly sensitive and emotive. The principles of data protection, dignity and confidentiality will underpin all communication.

·ÌýÌýÌýÌýÌýÌýÌýÌý Represent UHCW at external meetings with stakeholders.

·ÌýÌýÌýÌýÌýÌýÌýÌý Lead on virtual ward rounds with the wider multi-disciplinary team.

·ÌýÌýÌýÌýÌýÌýÌýÌý Confidentially communicate with patients, carers and family.

·ÌýÌýÌýÌýÌýÌýÌýÌý To be able to sensitively communicate bad news

·ÌýÌýÌýÌýÌýÌýÌýÌý Adopt and promote the highest level of verbal, non-verbal, written and electronic communication skills. Be open and honest, act with integrity and uphold the reputation of the profession as directed by the Nursing Midwifery Council (NMC, 2015).

·ÌýÌýÌýÌýÌýÌýÌýÌý Take responsibility for initiating and/or engaging in open discussion with statutory and voluntary agencies and all members of the wider multi-disciplinary team as appropriate

·ÌýÌýÌýÌýÌýÌýÌýÌý Ensure effective communication to support continuity of care on transfer between hospital, community, residential and nursing care settings and vice versa.
