Job overview
The role is to support Band 6 nurses in the Dudley Special schools and the Community Children鈥檚 nursing team. The team works as part of the Community Children鈥檚 Nurse Team (CCN Team) at Brierley Hill Health and Social Care Centre The nurse will support in managing the health needs of children and young people with complex health and profound learning disabilities.听
听
Main duties of the job
听The nurse will support in managing the health needs of children and young people with complex health and profound learning disabilities.
Working closely with the MDT to ensure that Education, Health and Care Plans are implemented. Some home visits and other community visits will be carried out as part of a holistic case management.
Provide clinical skills and ensuring a holistic, family centered approach to assessing, planning, implementing, and evaluating the health needs of the children.
Teaching/training and supporting the team as well as the school staff to ensure the health needs of the children are met,
Implementing strategies to support the care of children with learning disabilities.
The nurse will require the experience and ability to be able to manage emergency health situations in the school听

Detailed job description and main responsibilities
Main Duties/Responsibilities
听
飩诽 Demonstrate empathetic communication skills when dealing with patients and carers along the patient pathway.
听
飩诽 Provide specialist nursing advice and support to families of children with significant medical needs 听under the direction of the band 6 in the special schools
飩诽 Complete home visits as required supporting the families and children and assess the child鈥檚 clinical condition
飩诽 Undertake specific clinical tasks and skills for which the nurse has received
training and is deemed competent in all clinical settings. This will include, suctioning, oxygen management, tracheostomy care, care of central lines and vascuports and other clinical procedures
飩诽 Encourage children and families to participate in their care by providing training and support on a range of clinical procedures to meet the health needs of the child within
scope of expertise
飩诽 Working 听in 听conjunction 听with 听the 听band 听6 听and 听other 听agencies, 听implement 听agreed transition of care processes ensuring it takes place in a timely fashion and meets the ongoing needs of the 听child/young person
飩诽 Have knowledge of the SEND agenda and EHCP鈥檚 and contribute to EHCP鈥檚 and the
reviews to ensure that the optimum support and care is provided for the child and family
飩诽 Clinically assess the child or young person and ensure that actions are taken to promote safety
飩诽 Support the safe administration of medications within the school and support staff to be trained adequately and that their competencies are assessed.
飩诽 Provide and maintain a high standard of skilled nursing care /skills for children with additional听 听health听 听needs.听 听Facilitate听 听care听 听at 听home 听for听 听a听 听children 听with听 听short term/complex health conditions including children who are palliative
飩诽 Provide reports, as appropriate for CYP鈥檚 EHCP reviews
飩诽 Demonstrate knowledge, skills, competency and innovative practice in paediatric nursing Care
飩诽 Demonstrate continual evaluation of clinical practice and recommend changes
飩诽 Assist in the completion of detailed holistic assessments of children and families, while giving consideration to cultural differences.
飩诽 Ensuring that the changing needs of individual children and families are identified
and that adjustments are made to the child鈥檚 care plan.
飩诽 To establish or maintain relationships with the child and family where relevant whilst the child is an inpatient in hospital to help facilitate smooth discharge.
飩诽 Completing accurate and timely recordings of involvement with the child and family and reporting any changes affecting the child and family appropriately.

飩诽 Work in conjunction wither other nurses in the 听team and provide cover as required depending on scope of practice

飩诽 To maintain an electronic portfolio of evidence that offers assurance against CQC
essential standards and revalidation.
飩诽 Line Manage unregistered staff in the school if required- supported by band 6
飩诽 If required take responsibility for the management of the school in the absence of the band 6
飩诽 To work under the overall direction of the Band 6, but maintains accountability and responsibility for own work.
飩诽 Inform line managers of any issues affecting safe and effective service delivery at the time an issue is highlighted.

Educational/training

飩诽 Under the direction of the band 6 provide training for the schools about complex medical needs/disabilities and the support services available for these children. This will include information about individual children鈥檚 conditions, and general training about medical needs.
飩诽 To facilitate training and implement teaching plans for parents, school staff to
empower them to manage a child/young person鈥檚 condition
These may be formal or informal educational initiatives
飩诽 Assess the competencies of school staff and delegate care tasks as appropriate to education staff
飩诽 Maintain knowledge and competence in computer skills, in order to access 鈥渄rives鈥,
databases, email and approved internet resources and critically appraise sources of information and their evidence base.
