Job overview

Emergency Medicine is evolving rapidly. The UK population is ageing and the number of older and vulnerable patients attending urgent and emergency services is increasing. There are well documented challenges to managing patient flow, reducing the length of time patients stay in EDs and keeping them safe when the service is under pressure. Our clinical experience and the evidence tells us that long periods in ED leads to increased morbidity and mortality, especially for older people with frailty.

At UHSussex we have a team of dynamic and enthusiastic colleagues, exploring different ways of working and addressing these challenges, and who are committed to making improvement. We are already improving flow and avoiding hospital admissions. We are not the finished article, but we are improving.

This post gives a higher specialty level resident doctor an opportunity to make a real difference to the care of older people with frailty in the emergency department.

This fellowship will involve working with patients, relatives, staff members and senior leaders in the geriatric and emergency medicine departments to develop and embed a toolkit and resources to guide the interprofessional workforce ensuring safe effective care for fail, elderly vulnerable patients. This toolkit would be accompanied by the development of an education package for emergency department staff.

Main duties of the job

The postholder has a unique opportunity to learn leadership skills in real life in one of the most challenging areas of NHS practice, with support from the BSMS leadership mentorship programme and a PG Certificate, plus supervision from both geriatric and emergency medicine consultants.

The leadership part of the job is 60% of the time, the postholder will also work clinically for 40% of their time.

This role will include:

  • working with key stakeholders to develop a toolkit for the care of older people in the ED setting
  • embedding the toolkit to guide the interprofessional workforce
  • delivering an associated education package
  • implementing other changes resulting from stakeholder engagement
  • evaluating and disseminating changes
  • working clinically across the emergency department and frailty SDEC to manage older people with frailty syndromes at a higher specialty training level (ST3-ST7)
  • academic and personal development through completion of the bespoke BSMS/deanery leadership development programme, including PG certificate in Healthcare Leadership and Commissioning

Person specification

Experience and Qualifications

Essential criteria
  • Basic Medical Degree
  • MRCP/MRCS or appropriate specialist qualification
  • Full GMC Registration with a licence to practice
  • Advanced Life Support Certificate from the Resuscitation Council UK or equivalent
Desirable criteria
  • Higher postgraduate degree

Clinical Skills and Experience

Essential criteria
  • Experience of geriatric or emergency medicine at HST level (ST3-7)
  • Able to complete a comprehensive geriatric assessment

Organisation and Planning

Essential criteria
  • Experience and ability to work in multi-professional teams
  • Understanding of: NHS / Clinical governance / Resource constraints
Desirable criteria
  • Evidence of managerial skills: Achievements / Course attended
  • Experience of quality improvement projects

Teaching Skills
