Main duties of the job
To be responsible for the assessment, planning, implementation and evaluation of evidence based individualised plan of care. The post holder will also assist in the management and organisation of nursing work in the ward/department.

Working for our organisation
We employ more than 6,500 staffÌýwho deliver compassionate care from our two main hospitals,ÌýCalderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, healthÌýcentres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.

Detailed job description and main responsibilities
- Maintain a high standard of patient focused care in accordance with Calderdale & Huddersfield NHS Foundation Trust policies and procedures
- Maintain a safe and clean working environment
- Provide assessment, planning, implementation and evaluation of nursing care for patients, ensuring accurate written records are kept.
- Communicate with patients and carers to ensure they have access to appropriate information.
- Promote and maintain positive relationships between all staff involved in the care of the patient
- Act in accordance with the Nursing and Midwifery Council Code of Conduct
- Undertake duties on different wards/departments as and when required by service demands
- Demonstrate personal responsibility for compliance with infection control policies and procedures

Qualifications/Training
Essential criteria
- NMC Registered Nurse in relevant field of practice (or within 6 months of completing a validated BSc or MSc course, resulting in NMC registration)
- Practice Assessor/Practice Supervisor qualification or willingness to complete
Desirable criteria
- Additional post-registration qualification relevant to field of practice
- Evidence of continuing professional development
- Role Specific Training relevant to specialty of the post

Knowledge, experience & expertise
Essential criteria
- Knowledge and skill to plan, deliver, and evaluate high quality, evidence-based nursing care to a group of patients within the speciality of the post
- Experience of working with patients, and those that matter to them, from a range of diverse cultures and backgrounds
- Experience of working as part of a team, with the ability to prioritise own workload.
- IT literacy and ability to use NHS systems
Desirable criteria
- Clinical experience in the specialty of the post

- Excellent interpersonal and communication skills with the ability to adapt to meet the requirements of patients, their families/carers, and professional colleagues.
