Care coordination is not one person鈥檚 role, job or responsibility. It is the joining up of services, coordination, information and communication between care givers, treatment providers, those living with and beyond cancer which is referred to as personalised care and support and their families that creates a seamless experience of care. Reporting to the Lead Cancer Nurse the post holder will be responsible for operational management and leadership of the Cancer Support Worker (CSW) team and working with the wider clinical workforce will support the implementation of Personalised Care and Support (PCS) agenda for all cancer patients with the trust.
In conjunction with the Lead Cancer Nurse the post holder will contribute towards the Personalised Care and Support (PCS) strategy.
In conjunction with the Lead Cancer Nurse, the post holder is required to co-ordinate and support delivery of the cancer support services across all tumour sites, ensuring there is equity for all patients affected by a cancer diagnosis.
The post holder will line manage the Cancer Support Workers.
Main duties of the job
As the Cancer Support Worker Team Lead, the post holder will be responsible for managing a Team of Cancer Support Workers, working alongside the MDT in promoting patient self-care, supporting patients diagnosed with cancer to regain as normal a life as possible post diagnosis and treatment.听
The post holder will undertake a scoping exercise to support the Lead Cancer Nurse and Lead Cancer AHP understand what the future opportunities are for the service.听

Essential criteria
- A relevant degree or equivalent knowledge and experience
- Willingness to undertake learning and development courses
- Evidence of ongoing personal professional development

- Previous experience within cancer support worker role
Experience

- Experience in a supervisory role
- Experience in prioritising and organising own workload
- Previous experience in the NHS or Social Care
- Experience of leading a project
- Experience of maintaining comprehensive record keeping

- Experience of presenting information to a range of staff
- Relevant health or social care experience at AfC band 4 or equivalent
- Experience of multi-professional working

Knowledge and Skills
Essential criteria
- Working knowledge of the Personalised Care and Support Programme
- Excellent working knowledge and experience of Microsoft Office and Windows systems

Applicant requirements
The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
