B6 Senior Occupational Therapist Frailty

Job overview

Are you a highly motivated, experienced and committed聽 Occupational Therapist (OT) with a passion for working with older patients and those with frailty? Then come and join the Older Person's Therapy Team at Salisbury District Hospital.

The successful applicant will be responsible for the delivery of high quality, patient-centred care on our award winning Acute Frailty unit (AFU) The unit provides specialist holistic assessment, care and treatment for frail patients in line with Same Day Emergency Care (SDEC)聽 principles. Working with the Multi Disciplinary team, you will ensure rapid assessment, care and discharge planning to enable short lengths of stay, efficient flow and聽 prevent hospital related deconditioning.

You will also provide senior OT support to our new older person's ward which has been designed to cater for the complex needs of older people in a state of the art, dementia friendly environment.聽

The team also cover 2 other older person's wards, a Haematology ward and Falls Clinic. All of which will benefit from your skills, experience. and senior support.

The successful applicant will become an integral part of a kind, friendly and hardworking聽 team and will join our senior leadership team to help to lead, design and develop the service of the future.

Main duties of the job

The role will involve:

  • Early assessment and collection of social and collateral social history
  • Development of care and manual handling plans
  • Prescription and provision of equipment
  • Mental Capacity and Best Interest Decisions
  • Enabling the very best of End Of Life Care at home if this is the patient's wish
  • Teaching and supervision of Junior members of staff and OT students
  • Supporting the leadership team in the day to day running of the service

Qualifications

Essential criteria
  • BSc honours degree or diploma in Occupational Therapy
  • Full registration with the Health & Care Professions Council
Desirable criteria
  • PQ study relevant to post

Training and Experience

Essential criteria
  • Evidence of complex case management & problem solving approach
  • Broad clinical experience as a Band 5 therapist with some understanding or experience of working within a community setting
Desirable criteria
  • Evidence of successful working within a community setting

Planning and Organisation skil

Essential criteria
  • Ability to support more junior staff in caseload co-ordination, organising, delegating and prioritisation.
  • Evidence of being self -motivated with excellent organisational skills, planning skills and time management skills.

Other skills

Essential criteria
  • Evidence of involvement in project work or service development
  • Evidence of understanding & experience of working across organisational boundaries
Desirable criteria
  • Experience of supervising junior staff and/ or students
